Primary Duties:
Clinical:
Assessing, planning, providing and evaluation of individualised patient care, which is safe, evidence based and within the nurses' scope of professional practice.
Promote health and wellbeing, giving general advice on diet, exercise, alcohol consumption and smoking cessation.
Enable patients to manage their own health needs.
Where required undertake investigations, evaluate test results, and act upon or escalate them to the appropriate clinician as required.
Work within and support development of nurse protocols as required.
If Nurse Prescriber, it is the nurses responsibility to adhere to national and local guidelines, protocols and best practice associated with prescribing elements of their role
Long term conditions management, primarily diabetes focus but this may also include CHD; Respiratory conditions; CKD and hypertension.
Maintain stock of drugs and equipment reporting to nurse management of equipment failure/maintenance requirements, and developing lean working practices.
Contribute to clinical data collation and reporting as required.
Contemporaneously record all Patient Contact information on the practice medical record system.
Learning Facilitator Role
Promote a learning environment for patients and other health professionals.
Act as a Sign off mentor to more junior members of the team, assessing competencies against set standards/ NMC regulation/internal and external policy.
Support the operational Nurse Manager in facilitating pre-registration learners undertaking placements at Priory Medical Group.
Offer expert nurse advice to more junior members of the team on a day to day basis.
Providing support to more junior members of the team in their learning journey.
Provide information about clinical practice to the Lead Nurse as part of the 360 degree appraisal of the nursing team
Facilitate the nurse team undertaking required role appropriate training both in the practice and on external accredited courses.
Work with the Lead Nurse to develop strategic plans for workforce development.
Work with the operational manager to review and develop Post registration nurse competency framework.
Support the Lead nurse in development of, implementing and evaluating infection control policies in the practice.
Education:
This role required level 1 nursing registration with the NMC and it is the post holder's responsibility to maintain all relevant elements of their professional registration.
The employee will be responsible for developing and sustaining own knowledge, clinical skills and professional awareness in accordance with PREP.
To provide ongoing advice and information to patients about their condition or provide referral to other primary or secondary care providers if necessary.
Where appropriate, prepare results of audit and present as posters or scientific presentations at meetings and conferences.
Secondary Clinical Duties
1. Routine cervical screening and contraceptive initiation and management.
2. Conduct New Patient Health Checks
3. Wound Management and wound care planning including removal of Sutures or clips
4. Management of minor injuries/ailments
5. Childhood immunisations
6. Routine vaccinations and injections
7. Assist with or undertake coil and Implanon fitting and removal dependent upon training and skills.
Clinical Care Administration
- Patient Records - Accurate, efficient and appropriate Record Keeping
- Use of templates Accurate, efficient and appropriate use of clinical templates
- Protocols - Accurate, efficient and appropriate use of protocols
- QOF - Accurate, efficient and appropriate recording of clinical data as required for chronic disease management.
