Job summary
CHAW Primary Care Network (PCN), hosted by Wilmslow Health Centre, is seeking a proactive and compassionate Care Coordinator to join our growing multidisciplinary team.
This is an exciting opportunity to work across a collaborative network of GP practices covering Chelford, Handforth, Alderley Edge and Wilmslow. You will play a key role in supporting patients, particularly those with complex needs, to navigate services and receive coordinated, high-quality care.
As a Care Coordinator, you will be at the heart of improving patient outcomesworking closely with clinicians, care homes, and community services to ensure care is joined-up, timely and patient-centred. No two days are the same, and you will have the opportunity to make a real difference to peoples lives.
This role is funded through the Additional Roles Reimbursement Scheme (ARRS) and includes a fully supported, mandatory 5-day training programme to help you succeed and develop within the role.

Main duties of the job
As a Care Coordinator, you will support patients and clinical teams by helping to organise and streamline care across services. You will act as a key point of contact, ensuring patients, carers and professionals are kept informed and supported throughout their care journey.
The role involves coordinating activities across GP practices, care homes and community services, helping to ensure timely interventions and continuity of care. You will contribute to multidisciplinary team working, including supporting meetings and follow-up actions.
You will also support essential administrative functions, including managing document workflows, scanning, coding and processing clinical correspondence, and maintaining accurate patient records.
This is a varied and patient-facing role, requiring strong communication, organisation and a proactive approach to problem-solving.

Job Purpose
As a Care Coordinator, you will be a key member of the Primary Care Network (PCN), supporting the delivery of high-quality, patient-centred care through effective coordination of services and multidisciplinary working.
You will work closely with GP practices, care homes, and community services to coordinate care for patients, particularly those with complex needs, long-term conditions, or requiring MDT input. You will act as a central point of contact for patients, families, and professionals, ensuring care is seamless, proactive, and well-managed.
Key Responsibilities
Care Coordination & PCN Delivery

Support delivery of PCN DES requirements and priorities

Coordinate care for care home residents and patients following hospital discharge

Support PCN programmes including cancer, screening, SMR, and enhanced care home services

Assist in monitoring performance and supporting achievement of PCN targets

Contribute to service development, including visiting services and extended access
Multidisciplinary Team (MDT) Support

Organise and coordinate MDT meetings, including virtual care home ward rounds

Prepare agendas, identify patients, and circulate relevant information

Record actions/minutes and ensure accurate documentation in clinical systems

Liaise with MDT members to support integrated working
