Were looking for a proactive pharmacist to enhance personalised care and medicines optimisation within a dynamic multi-disciplinary setting. Whether you're already an Independent Prescriber or on the path to becoming one, well support your growth through the Primary Care Pharmacy Education Pathway (PCPEP) or IP qualification.

Youll work closely with GPs, ANPs and care coordinators to support chronic disease management, improve prescribing safety, and empower patients through expert medicines advice. You'll have protected development time, clinical autonomy, and the chance to shape services that make a real impact.

Main duties of the job

As a Clinical Pharmacist, youll play a central role in delivering safe, effective, and personalised care across our patient population. You will conduct structured medication reviews, support long-term condition management, and provide expert advice on medicines to patients and clinicians. Your day-to-day work will include reviewing prescribing for safety and efficacy, managing repeat prescriptions, and contributing to audits and quality improvement initiatives.

Youll also liaise closely with GPs, nurses, and the wider team to manage clinical queries, optimise prescribing, and reduce medicine-related harm. In time, or as an Independent Prescriber, youll be able to run clinics for specific conditions such as hypertension, asthma, or polypharmacy reviews.

You will have protected time for development, and youll be part of a supportive team where your clinical judgement is valued and your ideas can influence service delivery.

We are seeking a proactive and motivated Clinical Pharmacist, ideally an Independent Prescriber or working towards Independent Prescribing, to join our clinical team. You will play a key role in ensuring safe, effective, and personalised use of medicines across the practice population.

This is a great opportunity to contribute to service improvement, enhance medicines optimisation, and directly support our GPs and wider clinical team in delivering patient-centred care.

Conduct Structured Medication Reviews (SMRs) for patients with polypharmacy, frailty, long-term conditions, and/or complex needs

Support chronic disease management including hypertension, asthma, COPD, and diabetes

Provide advice and clinical support on medicines to GPs, ANPs, nurses, and practice staff

Review and manage repeat prescription requests to ensure appropriate, cost-effective prescribing

Reconcile medications post-discharge and after outpatient visits

Contribute to QOF, IIF and other quality initiatives

Support and implement practice-level audits and prescribing improvement projects

Respond to medication-related queries from patients, pharmacies, and colleagues

Assist with medicines safety monitoring and drug alerts

If an Independent Prescriber, run condition-specific clinics and initiate medications within your scope

Work collaboratively with the wider Primary Care Network (PCN) where required
