Within the Team Up service, care coordinators fulfil a vital
administrative function, overseeing the organisation and management of client
care pathways across health and social care systems. Their role is central to
streamlining processes, ensuring accurate documentation, scheduling
appointments, and coordinating communication between various professionals and
agencies. By handling these essential administrative tasks, care coordinators
help reduce inefficiencies and support the smooth operation of services, making
it easier for clients to receive timely and appropriate support. The working hours for the role are between 1pm and 6.30pm.

Act as the first point of contact for patients,
carers, and professionals contacting the Team Up service.
Coordinate patient appointments, follow-ups, and
referrals, ensuring efficient scheduling and communication.
Maintain accurate and confidential records, both
electronic and paper-based, in line with data protection and information
governance policies.
Support the multidisciplinary team with
administrative tasks including minute-taking, report preparation, and
correspondence management.
Assist with the monitoring and reporting of
patient care activity, identifying areas for improvement and supporting service
development initiatives.
Communicate effectively with patients, their
families, and healthcare professionals to ensure a patient-centred approach to
care coordination.
Manage incoming calls, emails, and queries,
providing information or signposting as appropriate.
Contribute to the development and implementation
of administrative processes to enhance service delivery.
Participate in regular team meetings and
training sessions as required.
