Glossop Primary Care Network (PCN) are looking to recruit a full time Advanced Nurse Practitioner with experience in primary care to join our team in providing a high quality service to our patients.Prescribing qualification desirable but not essential.

Main duties of the job

The Advanced Clinical Practitioner (ACP) will work autonomously, managing a caseload of patients with complex health needs, providing assessment, diagnosis, treatment, and management plans, and ensuring continuity of care in collaboration with the wider multidisciplinary team.

This post supports the PCNs commitment to caring patients close to their homes, reducing hospital admissions, improving patient outcomes, and enhancing the overall patient experience within the community setting.

The role will consist of a community delivery of healthcare through acute home visits, and engaging proactively with population health as a part of the Team Up initiative.

Team Up Service Key Features

  • Operates Monday to Friday from 8am to 6.30pm: Colleagues must be flexible to ensure cover during service open hours. This includes covering team absences as agreed by both parties.
  • Integrated Care Model: Brings together professionals from health, social care, and community services including advanced clinical practitioners, district nurses, social workers, home carers, and GPs to form one cohesive team.
  • Home-Based Support: Focuses on delivering care directly in patients homes, reducing the need for GP visits, emergency department attendances, and hospital admissions.
  • Preventative & Proactive Care: Aims to keep vulnerable individuals safe and well at home, preventing crises and supporting hospital discharge.

Glossop PCN consists of 4 practices in Glossop and Hadfield who have a very cohesive working relationship. We cover a population size of 33,500 patients. Clinical Assessment and Management: Conduct comprehensive clinical assessments of patients in their homes, utilising advanced clinical skills and evidence-based practice to diagnose and manage acute, chronic, and complex conditions.

Autonomous Decision-Making: Exercise professional judgement to initiate appropriate investigations, interpret results, and develop individualised care plans, including prescribing medications within scope of practice.

Care Coordination: Collaborate with GPs, community nursing teams, mental health professionals, social care, and voluntary sector partners to ensure seamless care and appropriate referrals.

Patient Education: Provide education, advice, and support to patients and their families or carers to promote self-management, independence, and wellbeing.

Emergency Response: Identify and respond to deteriorating patients, escalating care as appropriate, including arranging urgent hospital admissions when necessary.

Documentation: Maintain accurate, timely, and comprehensive clinical records in line with professional and organisational standards.

Quality Improvement: Participate in audit, service evaluation, and quality improvement initiatives to enhance patient outcomes and service delivery.
