Job summary

We have an exciting opportunity for a Paramedic to join our team. As a paramedic in a busy GP surgery you will need to work within the Surgery using your skills and clinical judgement to provide a first point of contact for our patients that present to us with minor illness/conditions.

Main duties of the job

To support and work alongside the wider clinical team, dealing with all the acute/on-the-day patients. This role will involve assessment, diagnosis and treatment of these patients who present with undiagnosed conditions.

Sparcells Surgery is part of the Sparcells PCN in Swindon, Wiltshire. The practice benefits from a multidisciplinary team of GP's, ANP's, nurses, clinical pharmacists, first contact physiotherapist and social prescriber. We care for over 10,000 patients. We have a very good team ethic at the practice and all work together to ensure high quality care is given to our patients.

MAIN ROLE AND PURPOSE OF THE POST

The Paramedic will work in collaboration with the multidisciplinary clinical team, to provide service to patients. They will work as an autonomous, accountable practitioner, in the provision of a holistic model of care. The paramedic will assess, diagnose, treat, refer or signpost patients who attend the practice with undifferentiated or undiagnosed conditions.

The post holder must own vehicle for home visits with expenses remunerated in overall financial package.

SPECIFIC RESPONSIBILITIES

Clinical responsibilities

Be accountable for decisions and actions via HCPC registration, supported by a professional culture of peer networking/review and engagement in evidence-based practice.

Ensure clinical practice is safe and effective and remains within boundaries of competence.

Assess, diagnose and treat patients in accordance with your training and competency. Where appropriate refer patients to colleagues.

Seek advice from secondary care using the Cinapsis system where necessary.

Refer patient to external organisations in accordance with national and local guidance. Advice should be sought if they do not meet the criteria.

Collect health measurements and educate patients on leading a healthy lifestyle.

Communicate effectively and appropriately with patients and carers, sharing complex and sensitive information regarding diagnosis, pathology and prognosis.

Educate patients, promotion self-care and empowering them to make informed choices about their treatment.

Undertake home visits to patients who are acutely unwell to assess and diagnose their condition. Agree a treatment plan with input from other clinicians as appropriate.

Record all contacts in SystmOne in an accurate and timely manner, using appropriate clinical coding.

Prescribe appropriately in accordance with your competence and national and local guidelines.

Ensure that all Infection control guidelines are followed.

Carry out regular clinical audit.

To formally and informally impart knowledge and skills to colleagues promoting peer review and best practice within the work environment.

Ensure evidenced-based care is delivered at the highest standards

Professional and administrative responsibilities

Maintain their competence and knowledge through continued personal development and adhere to all national policies regarding treatment.

Communicate effectively at all levels within the practice, ensuring an effective service is delivered.

  • Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans.
  • Demonstrates accountability for delivering professional expertise and direct service provision.
  • Exercise a critical understanding of personal scope of practice and to identify when a patient needs referring on.
  • Through patient assessment and working in partnership with patients and their carers, make decisions about the best pathways of care, informed by the urgency and severity of patient need, patient acuity and dependency, and the most appropriate deployment of resources.
