Job summary

We are looking for an enthusiastic and efficient individual to join our busy but friendly team who has the skills and competencies to provide a high quality clinical coding service to support the practice.

Main duties of the job

The main responsibility is to screen all incoming medical letters, entering key data and selecting and directing appropriate information for GPs attention as per practice policy and protocols. The role will involve electronically scanning, filing and summarising to patients medical notes. Coding incoming clinical information and summarising past medical history, investigations, problems and ensuring long term conditions are identified and embedded within the practice recall system.

Working directly with the IT/Admin Lead and Practice Manager the role requires a high level of concentration and attention to detail particularly with aspects such as data entry, summarising and data analysis.

About us

We are a 4 partner GP Practice based within the Whitefield/Unsworth area of Bury.

Our friendly team comprises of both clinical and non- clinical staff (practice nurses, healthcare support worker, advanced clinical practitioner, admin and reception), working collaboratively with our colleagues across the Primary Care Network (physio, pharmacists, multi-disciplinary team co-ordinators).

We are a training practice and currently have 3 GP trainee doctors and 5th year medical students.

Job description

Job responsibilities

Review all incoming correspondence filing the document and entering relevant information into the patients electronic medical record using appropriate codes in accordance with practice policy/protocol.

To understand and maintain the need for accuracy of clinical coding as this has a direct impact on the revenue generation and funding for the practice.

Highlight actions and workflow to the clinicians as per policy/protocol within an agreed timeframe

Accurately code data on to the EMIS clinical system

Identify new diagnosis and code to long term condition registers ensuring patients continuity of care within the recall and diary system

Undertake the process of note summarising as per practice policy

Work with the Practice Manager and IT/Admin Lead to monitor data analysis for the purposes of achievement of Quality and Outcomes Framework

Data cleansing and quality assurance including correction of anomalies in patient records

Organise daily workload so that personal coding targets are met and support the practice team in achieving high standards of coding completeness and accuracy within agreed timescales.

Ensuring accurate, timely and safe processing and reporting concerns to the Practice Manager

Support the IT/Admin Lead in audit data collection

Receive, process and disseminate private work to the clinicians and liaise with patients accordingly.

Qualifications

Essential

  • Minimum GCSE grade A to C in English and Maths
  • Evidence of medical terminology or clinical coding

Desirable

  • National clinical coding qualification

Experience

  • Experience in clinical coding
