Job summary

Dr Gilkar & Partners are recruiting an advanced care or nurse practitioner to join our friendly & professional clinical team.

Main duties of the job

Use initiative and take responsibility for themselves and the quality of their work and the service they provide to patients.

Work with a high degree of autonomy and complex decision making underpinned by a masters level award that encompasses the four pillars of clinical practice, leadership and management, education and research, with demonstration of core capabilities and area specific clinical competence.

Act as a source of clinical expertise, knowledge and skills in advanced clinical practice that is accessible, flexible and provides a high-quality service to enhance patient care.

Take a lead role in the advanced clinical assessment of patients who have highly complex and or chronic disease presentations, including screening patients for disease risk factors and early signs of illness, making a differential diagnosis and prescribing treatments as an independent non-medical prescriber.

Monitor and review those that are at high risk of admission into an acute hospital setting with a view to reduce inappropriate unplanned admissions and A&E attendances.

Job responsibilities

KEY RESPONSIBILITIES:

Act as the first point of contact exercising clinical and professional autonomy involving critical judgement and expertise in providing clinical assessment and interpretation of clinical findings to ensure that the most appropriate care is delivered to patients referred with complex, urgent or chronic health care needs.

Prioritise health care needs and intervene appropriately to assist the patient in complex or urgent care situations and escalate as needed.

Develop, implement and evaluate individual plans of care with patients and their carers/relatives according to their current and changing health care needs and differential diagnosis to enable step up or step down to appropriate services when their condition requires.

Participate in MDT discussion and establish robust lines of communication with the patients GP to ensure contemporaneous feedback on treatment and interventions and facilitate high quality, safe, effective care.

Integrate both drug and non-drug based treatment methods into a care management plan with the support of medical practitioners and provide a range of treatments using patient group directions or non-medical prescribing, following clinical assessment and diagnosis.

Request diagnostic investigations and interpret within agreed protocols to ensure the most appropriate care/treatment is delivered.

Independently prescribe and review medications for therapeutic effectiveness, appropriate to patient need and in accordance with evidence based practice and national and local policy and protocols.

Work in partnership with patients, Doctors, other health and social care professionals, carers and relatives and the voluntary services sector in the planning and delivery of care and the safe transfers of care between organisations.

Practice within the NMC / HCPC / GPhC Code of Conduct and scope of professional practice.

Demonstrate critical thinking and analytical skills incorporating critical reflection.

Analyse and interpret clinical history, presenting symptoms, physical and psychological findings and diagnostic information along with physical examination to reach a differential diagnosis.

Recognise the limits of their scope of professional practice and know when to refer to the patients GP.

Communicate complex patient related information effectively to ensure integrated care working across health, social, private, charitable and voluntary care sectors.

Communicate effectively with patients and carers from all cultures to maximise their potential for recovery. This will frequently involve communicating using verbal and non-verbal methods with patients who have complex physical, emotional or psychological conditions.

Develop and maintain effective communication systems to ensure the staff in the integrated care team receive and contribute information regarding Trust and professional issues and developments.

Ensure high standards of patient care ensuring all patients' individuality and dignity are maintained at all times.

Facilitate open and harmonious communication amongst all staff within the team as well as with other departments with the aim of promoting multidisciplinary approaches to care.

Support others to develop knowledge and skills.

Identify need for change, leading innovation and managing change, including service improvement.

Collect, collate, evaluate and report patient information, maintaining accurate and contemporaneous records.
