Job summary
Recording contemporaneous consultations using EMIS.Provide safe, evidence-based, cost-effective, individualised pt care. Clinically examine - assess pt needs, plan clinical care accordingly referring pts to other services in a timely manner. Recognise the need for, be able to perform - interpret investigatory procedures, inc results. Prioritise health problems, intervene appropriately to assist the pt in complex, urgent or emergency situs, inc initiation of effective emergency care. Ensure appropriate f-up of pts. Support pts to adopt health promotion strategies that promote healthy lifestyles and self-care. Support the delivery of anticipatory care plans. Independently prescribe within your scope of practice in accordance with locally - national agreed guidelines. Compiling and issuing computer-generated acute - repeat prescriptions. Review meds following the appropriate policies, NICE guidelines - local clinical pathways. Awareness of - compliance with all relevant practice policies e.g. prescribing, confidentiality, data protection, health and safety and QOF standards. Be aware of your responsibilities regarding current legislation and adhere to our policies Safeguarding Children and Adults.
Main duties of the job
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
Assessing the health care needs of patients with undifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signs of illness
In consultation with patients and in line with current practice disease management protocols, developing care plans for health
Providing counselling and health education
Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
Recording clear and contemporaneous consultation notes to agreed standards
Collecting data for audit purposes
Compiling and issuing computer-generated acute and repeat prescriptions. Prescribing in accordance with the practice prescribing formulary or generically whenever this is clinically appropriate
