Summerhill
Surgery is a well-established local GP surgery in the Kingswinford area.
We
are looking for an additional member to join our Practice Nursing team due to
an increase in registered population.
The post holder is responsible for the
delivery of basic practice nursing services care to the practice population.
Supported by senior nurses within the Practice, they will deliver care within
the boundaries of their role, focusing upon supporting patients to be healthy,
monitoring of long-term conditions, health prevention and screening activities.
They
will work collaboratively with the general practice team to meet the needs of
patients, supporting the delivery of policy and procedures and providing nurse
leadership as required.
Main duties of the job
Assist with the organisation and
co-ordination of the provision of nursing services for the practice.
Provide nursing treatment to patients in
participation with general practitioners or independently agreed protocols,
including cervical smears.
Provide general and specific health screening
to the practice patients (within the agreed protocols) with referral to general
practitioners as necessary.
Be competent in administration of childhood
immunisation
and travel vaccination.
Be competent and confident in managing people
with long term health conditions
Assess, diagnosis, plan, implement and evaluate
interventions/treatments for patients with complex needs Proactively identify,
diagnose and manage treatment plans for patients at risk of developing a
long-term condition (as appropriate)
Prioritise health problems and intervene
appropriately to assist the patient in complex, urgent or emergency situations,
including initiation of effective emergency care
Support patients to adopt health promotion strategies
that promote healthy lifestyles, and apply principles of self-care.
Implement and participate in vaccination and
immunisation programmes for both adults and children
Meet the needs of patients presenting for
opportunistic wound care

Summerhill Surgery is a happy surgery trying to
provide the best possible service and have a patient focused ethos.
We currently have 7300 patients with approximately
15% living in Care Homes who are looked after by our specialist team.
We have 3 GP Partners and a non-clinical Managing
Partner with 3 further salaried GP's. Other members of our Clinical team
include ANPs, Nurse Practitioners, Practice Nurses, HCA's, a GP Assistant, a
Paramedic and Pharmacist support. We also have a large hardworking admin team
providing support to the clinicians and patients.

- Ability to work within own scope of practice and understanding when to refer to GPs
