Job summary

We are looking for a reliable, friendly person to join our admin team as a care coordinator working a minimum of 26 hours per week. Someone with experience of working in general practice and using EMIS Web and an understanding of clinical coding is desirable but not essential as full training will be given.

Recruitment will be through CV and interview. Please state in your application why you would like to work in Caterham Valley Medical Practice. Employment will be dependent on pre-employment checks including at least two references, completion of right to work checks and completion of a medical questionnaire prior to start date.

Main duties of the job

Main duties will be to support GPs with administrative tasks by managing clinical correspondence received into the practice and identifying a cohort of patients who would benefit from personalised care and support plans. The post-holder will form part of our multi-disciplinary, primary-care team.

Other duties will include leading on the practice screening programme, summarising of new patient records and managing appointments for new mothers and their babies.

Job description

Job responsibilities

Primary Responsibilities

a. Accurately managing clinical correspondence received into the practice.

b. Read coding appropriately into the patients clinical record.

c. Identifying relevant clinical information documented in patient correspondence and completing any detailed actions such as arranging appointments, referrals, tests and follow-up appointments.

d. Prepare documents for review by GPs as necessary.

e. Identifying patients to discuss at MDTs with a view to reducing unplanned admissions

f. Initiation and management of personalised care and support plans for a select group of patients identified using local knowledge and population health management information.

g. Using shared decision making to assist patients in making decisions about their care.

h. Sharing care plans with other parts of health and social care using appropriate methods.

i. Providing coordination and navigation for people and their carers across health and social care.

j. Liaising with the PCN pharmacists and social prescribing link workers/wellbeing team as required to support patient centred care.

k. Ensuring all practice care homes have a weekly check in call and regular visits.

l. Support GPs by being the first point of contact for general administrative tasks as requested.

m. Sorting, reviewing and read coding the medical records of new patients as per the practice summarising protocol ensuring an accurate summary of the patients past medical record is recorded in EMIS.

n. Leading on the practices screening improvement plan to ensure maximum uptake and reduction of DNAs.

o. Accurately recording screening results in patient records if results not automated.

p. Scheduling of post-natal checks for new mothers and 6/8 week checks and first immunisations appointments for new-borns.

q. Communicate effectively via email, telephone, written and face-to-face communication according to guidelines and customer care standards.

r. Ensuring timely and accurate handling of messages / tasks, ensuring all unresolved or urgent matters are prioritised.
