This is an exciting opportunity to work in a rural practice operating within the West Berkshire Rural PCN.
The ANP role is new to Lambourn Surgery. We have been fortunate to have an ANP for the last 12 months and are looking to recruit another to work alongside the existing team. They will form an integral part of our practice team and actively take part in clinical meetings and full staff meetings, feeding back ideas for improvement and change to the leadership team.
We have 3 clinical partners, all of whom have been at the surgery for a number of years. 2 of them have returned, having been registrars here during their training. The Senior partner has been here since 2005. The practice manager is also a partner, taking care of the business elements of the partnership and has been at the surgery since 2007.

Main duties of the job
Our ANP's form the duty team, working alongside a duty doctor and the minor illness nurse. They deal with most of the on the day demand and will consult face to face, over the phone or online. They may be asked by our team of care co-ordinators to triage requests for appointments where the urgency is unclear. They attend daily coffee meetings with the rest of the duty team and the GP assistant to discuss any complex patients or queries anyone may have.
You will be a prescriber and confident in prescribing using the most up to date guidelines.

Lambourn Surgery is in an area of outstanding natural beauty in a semi rural location. We serve a population of 6500 patients. Due to the rural nature of the practice we also dispense to 3000 of our patients. The village has a wide demographic and is famed for its horse racing which is evident as you drive over the downs to reach us. We are within 20 minutes drive of Swindon and Newbury and only 10 minutes from junctions 14 and 15 of the M4.
The entire team is very approachable and supportive of each other. We have regular staff outings and events, and hold regular team meetings to capture feedback and ensure all staff feel included in any major decisions around the running of the surgery.

Primary responsibilities
- Manage
patients presenting with a range of acute and chronic medical conditions, providing
subject matter expert advice
- Identify,
manage and support patients at risk of developing long-term conditions,
preventing adverse effects to the patients health
- Provide
advanced, specialist nursing care to patients as required in accordance with
clinical based evidence, NICE and the NSF
- Undertake the
collection of pathological specimens
- Request
pathology services as necessary
- Process and
interpret pathology and other test results as required
- Maintain
accurate clinical records in conjunction with extant legislation
- Ensure SNOMED
CT codes are used effectively
- Chaperone
patients where necessary
- Prioritise
health issues and intervene appropriately
- Support the
team in dealing with clinical emergencies
- Recognise,
assess and refer patients presenting with mental health needs
- Be an extended
and supplementary prescriber, adhering to extant guidance
- Support
patients in the use of their prescribed medicines or over the counter
medicines (within own scope of practice)
- Contribute to
practice targets (QOF, etc.), complying with local and regional guidance
- Liaise with
external services/agencies to ensure the patient is supported appropriately
(vulnerable patients, etc.)
- Delegate
clinical responsibilities appropriately (ensuring safe practice and that the
task is within the scope of practice of the individual)
- Support the
clinical team with all safeguarding matters in accordance with local and
national policies

practice and local policies for substance abuse and addictive behaviour,
referring patients appropriately
- Deliver
opportunistic health promotion where appropriate
Secondary Responsibilities
- Act as the audit
lead, effectively utilising the audit cycle
- Act as the IPC
lead for the practice, ensuring compliance, undertaking audit as necessary
- Monitor and
ensure the safe storage, rotation and disposal of medicaments
- Participate in
local initiatives to enhance service delivery and patient care
- Support and
participate in shared learning within the organisation
- Develop an area
of specialist interest, taking the lead within the organisation
- Continually
review clinical practices, responding to national policies and initiatives where
appropriate
- Participate in
the review of significant and near-miss events applying a structured approach
i.e., root cause analysis (RCA)
- Drive the
development of nursing services within the organisation, liaising with external
agencies and professional organisations as required
