Job summary
South Reading and Shinfield Group Medical Practice is a long established large GP Practice, rated GOOD in all categories by CQC. The Practice is located across two sites in South Reading. Our friendly, professional and experienced GPs, extended range of HCPs and administrative teams offering a wide range of clinical services.
We are seeking an enthusiastic Paramedic, ideally with experience working in Primary Care but this is not essential.
We are looking for a keen and committed Paramedic with excellent communication skills and clinical knowledge to join our multi-disciplinary general practice team. This post will be an exciting challenge for someone who is highly motivated and keen to develop their clinical and professional skills.
We are looking to utilise paramedic skills for home visits (for housebound patients, those in residential homes and to patients with pre-existing conditions who need treatment), urgent care clinics, treatment provision and triage.
Main duties of the job
CLINICAL
To practice as a care practitioner and demonstrate advanced clinical competence and knowledge above those associated with conventional nursing roles.
Assess, plan, treat and refer patients appropriately, carrying out consultations in the surgery or the community as an autonomous practitioner.
Using own clinical judgement to diagnose and treat patients either face to face consultation, clinical telephone triage and other mediums.
Making appropriate referrals to primary, secondary and social services care, following practice (and/or locally agreed) pathways and protocols.
To recognise the need for and initiate where appropriate relevant diagnostic investigations, using their skills in this area to take the correct course of action.
Understanding local safeguarding referral process and be aware of appropriate referral mechanisms for children and vulnerable adults
Ensure complete and accurate documentation of every patient contact, including advice given. Records must be accurate and reflect the clinical activity in accordance with NMC guidelines on clinical record keeping.
Minor illness assessment, diagnosis and treatment with access to seek advice, review and onward referral within clinical team.
To develop care plans for patients when required (as developed and supervised with the Partners)
Be involved with continuity of care of our patients, e.g. end of life care, chronic disease management.

We are a friendly, supportive, practice that has as a committed multi-professional team. We currently use the EMIS Web clinical system. Our Team aims to provide healthcare with courtesy, care and efficiency, to all patients.
