Job Title: Social Prescribing Link Worker (SLPW)

Reports to: SPLW Team Leader

Accountable to: Primary Care Network Operational Manager

Salary: Starting from £28,500 pro rata

Hours: 26 hours per week

Location: CHOC PCN Practices

Contract: Fixed Term to cover maternity leave (dates and timings not yet confirmed)

Main duties of the job

  • Manage a caseload of patient referrals for the social prescribing service, ensuring suitability and engaging patients in one-to-one discussions focused on their priorities and wellbeing.
  • Build trust and provide non-judgemental, inclusive support, helping patients identify wider issues affecting health.
  • Co-produce simple, personalised support plans based on individual needs, values, and motivations, outlining available services and self-care actions.
  • Support patients in achieving goals through regular follow-up and liaise with health professionals as needed, referring back when appropriate.
  • Promote social prescribing within the PCN and local agencies, fostering strong relationships and encouraging referrals.
  • Develop links with community organisations and partners to address unmet needs, create diverse service menus, and strengthen community resilience.
  • Assess and manage risk, including safeguarding and suicidal ideation.
  • Educate PCN staff on community services and access pathways.
  • Record data accurately, provide feedback to referrers, and ensure compliance with data protection.
  • Produce basic reports, maintain quality standards, and engage in ongoing training and appraisal for professional development.

About us

An exciting opportunity has arisen to join our Primary Care Network (PCN) social prescribing team on a fixed-term maternity cover contract.CHOC PCN covers practices across Congleton and Holmes Chapel, and is seeking an experienced, enthusiastic, and forward-thinking Social Prescribing Link Worker to join our established team.

This frontline role works closely with practices to empower patients to take control of their health and wellbeing, focusing on what matters most to them. Social prescribing connects people to community groups and statutory services for practical and emotional support, taking a holistic approach to health.

You will support a wide range of patients, including those with long-term conditions, mental health challenges, loneliness, isolation, or complex social needs. We are looking for someone who is a good listener, empathetic, and committed to helping local communities care for each other. Experience working positively with people facing complex social and emotional challenges is essential.

The successful candidate will work across all practices, delivering care to patients from diverse backgrounds. At the heart of this role is a commitment to timely, efficient, and confidential support.
