Job summary
The post holder will work as part of a friendly, motivated, multi-disciplinary team in a patient-facing role and will take responsibility for areas of chronic disease management. They will also be required to undertake structured medication reviews to proactively manage patients with complex polypharmacy and those with multiple co-morbidities.
The post holder will also provide primary support to general practice staff with regards to prescription and medication queries. They will deal with acute prescription requests, medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of the patient.
The post holder will also provide support on medicines optimisation and quality improvement programmes.
The post Holder will need to demonstrate an understanding of current educational policies relevant to working areas of practice and keep up to date with relevant clinical practice.
The post holder will maintain constructive relationships with a wide range of practice and PCN staff. The role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver excellent service within general practice.
Main duties of the job
- Undertake medication reviews and work within your scope of practice to implement any necessary changes (or produce recommendations for/refer to other clinicians to implement, if outside your scope)
- Undertake clinical audits of prescribing in areas identified internally by the Advanced Practitioner Clinical Pharmacist and practice and externally by quality scheme contractual requirements, feedback results and implement changes in conjunction with this
- Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance
- Managing caseload of patients while working within a scope of practice and limits of competence
- Signpost to community pharmacy and healthcare professionals
- Provide patient facing and telephone clinics to answer questions, queries, and concerns about patients medicines
- Reconcile medicines following hospital discharges or admissions, including identifying and rectifying changes and working to ensure patients receive the medicines they need
- Set up and manage systems to ensure continuity of medicine supply to high-risk patients
- Answer enquiries, suggest and recommend solutions. Provide follow up for patients to monitor effects of changes
- Ensure systems are in place for drug monitoring, streamlining these where possible
- Advise on the development and implementation of new services that have medicinal components

We are looking for Candidates to join our PCN Clinical Pharmacy team. We currently have an Advanced Practitioner Clinical Pharmacist, three Clinical Pharmacists and a Pharmacy Technician.
