This is an opportunity for an excellent administrator to join a team of care co-ordinators within Epsom PCN. It is an exciting prospect to join the team providing services at scale to offer holistic and proactive care.
Main duties of the job
Processing referrals for PCN services, booking patients as needed and communicating to patients, staff and practices alike.
Proactively working through searches to support QOF and medicines monitoring.
Supporting practices with patient registrations.
Working through CQC audits and other patient safety searches.
Forming close working relationships with our community teams, GPimhs team, care homes and practices.

Epsom PCN is part of Surrey Heartlands ICB. It is a largely urban area with some semi-rural populations and is
served by the London facing Epsom St Helier NHS Trust. Famous for its
racecourse and the annual Epsom Derby, Epsom PCN is within the Surrey Downs
Area of Outstanding Natural Beauty.
The PCN has a population of circa 62,500 patients served
by St Stephens House Surgery, Shadbolt Park House Surgery, Ashley Centre
Practice, Fountain Practice, The Derby Medical Centre and Spring Street Surgery. The Clinical Director is Dr Sam Eldred and she is supported by Operations Director Rachel Graville.
The PCN is committed to serve its patients, delivering
services in accordance with its population health demographic, accessible to
all and agile to change. The PCN has been awarded funding to test bed new products with the potential to deliver transformational change in primary care so it is an exciting time to join.

Job responsibilities
Managing patient records, handling correspondence, supporting appointment bookings, and ensuring smooth communication between patients, healthcare professionals, and external agencies.
The role requires a high level of accuracy, discretion, and professionalism, with the ability to manage multiple tasks and maintain patient confidentiality at all times.
Work collaboratively with GPs and other primary care professionals within the PCN to proactively identify and manage a caseload, which may include patients with long-term health conditions, and where appropriate, refer back to other health professionals within the PCN.
Support the coordination and delivery of multidisciplinary teams with the PCN.
Support PCNs in developing communication channels between GPs, people and their families and carers and other agencies;
Support practices to keep care records up-to-date by identifying and updating missing or out-of-date information about patients
Ensure compliance with GDPR and data protection standards.
Support recall and monitoring processes for chronic disease management, immunisations, and screening.
Participate in audits and quality improvement activities as directed.
