We are looking to recruit a part-time, fully qualified,experienced Advanced Clinical Practitioner to join our team.

The vacancy is for a part-time ACP, weekly contracted hours between 28 to 34 hpw, ideally Monday, Tuesday, Thursday afternoon & Friday's however, negotiable.

Main duties of the job

The Advanced Clinical Practitioner will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and potential home visits. The post holder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence. The Advanced Clinical Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner. The post holder will use skills, knowledge and competencies as a qualified Advanced Clinical Practitioner in order to be responsible and accountable for managing patient caseloads for treatments, referral, admission and discharging patients as appropriate.

We are a well-established practice in Bolton. Our team comprises of 3 GP Partners, 1 part-time salaried GP, 1 Advanced Nurse Practitioner, 3 Practice Nurses, 1 Assistant Practitioner, 1 Healthcare Assistant, Practice Manager, Assistant Practice Manager, supported by an organised well structured administrative & reception team. PCN additional roles within the practice include, MSK practitioner, Mental Health Practitioner, Clinical Pharmacist, Advanced Clinical Practitioner, Paramedic, Social Prescribers. We are a well organised, structured practice, dedicated to providing excellent clinical care. Our teams throughout the practice are friendly and supportive.

Job responsibilities

  • Work as part of a multi-disciplinary team to deliver safe and high quality care.
    • Evaluate and prioritise patient symptoms and provide general advice and reassurance in line with guidelines.
    • Triage and treat patients, performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.
    • Co-ordinate the planning and delivery of care ensuring patients and carers/relatives are involved.
    • Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
    • Diagnose and manage acute and chronic conditions
    • Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.
