Join our team of GPs and ANPs in the New Models of Care programme for the Northamptonshire Rural East & South (NRES) Neighbourhood.
Work within our practices across the neighbourhood to deliver high-quality, same-day care to our patients.
We're looking for GPs to join our Neighbourhood Access Service, part of the innovative New Models Programme. You'll work autonomously and as part of a multidisciplinary team within primary care to provide expert assessment, diagnosis, treatment, and evaluation for patients presenting with undifferentiated primary care presentations. In addition to same-day access work, you will be expected to incorporate a proportion of routine GP appointments into your working day.

Main duties of the job
As a GP in the New Models of Care programme, you will provide autonomous care for our patients presenting with undifferentiated presentations mainly in same day settings. You will demonstrate safe clinical decision making and provide expert care by undertaking initial history taking, clinical assessment, diagnosis, treatment and evaluation of care, including appropriate follow-ups.
Working with other GPs and nursing team in the neighbourhood Practices and Hub you will continually review and evaluate services and work with the management team in implementation of change.

About us
The Northamptonshire Rural East & South (NRES) Neighbourhood, consisting of 7 GP Practices serving 88,938 patients, is developing an innovative new model of care as part of the NHS New Models Programme.
You will be working with a team of GPs and specialist ANPs to deliver same day care to the Rural East & South population across our three hub sites.
We are pleased to offer flexible base locations across our seven member practices and three hub sites. We will work with successful candidates toidentifya primary location that best suits your preferences and our service needs.

Clinical
- Provide clinical care to our patients. This will include reducing the demand on the core neighbourhood GP practices by the assessment and management of patientsby means oftelephone triage and clinic appointments.
- Use advanced skills and expert knowledge to assess the physical andpsycho socialneeds of patients with undifferentiated primary care conditions including minor illness and instigating therapeutic treatments based on best available evidence to improve health outcomes.
- Use advanced skills and expert knowledge to make comprehensive and focussed assessments, independently and proactively managing care of the patients.
- Promote patient independence and self-care where appropriate
- Support and work cohesively in a team with other GPs/ANPs and ACPs working in the new model of care settings, including providing oversight and mentorship for more junior colleagues.
- Review pathology results to ensureappropriate treatmentand care
- Support and lead the practices for the delivery of the Quality and Outcomes Framework and NHS England/ICB/Federation Contracts
Regulatory
- Respond to complaints and clinical incidents/significant eventsasappropriate toensure robust systems are in place for improving practice through joint learning.
- Maintainaccurateandtimelyclinical records in line with professional and legal standards.
- Undertake at least one clinical audit a year. Evaluate data and present findings to the Practice Clinical team. Implement changes whereappropriate.
- Adhere to the employing practice Policies and Procedures including Infection Control, Health and Safety, and safeguarding. Ensure all mandatory training is completedin a timely manner.
- All staff must be familiar with and activelypromote the practices Equal Opportunities Policy at all times.
- All staff who have access to personal data will be aware of their responsibilities under UK GDPR and Data Protection Act 2018 and will abide by the principles of that legislation.
Personal / Professional Development
- Maintain own personal/professional development plan and portfolio thereby meeting the requirements for revalidation through appraisal of own performance.
- Adhere to the annual appraisal process and revalidation requirements.
