Job summary

We are seeking an enthusiastic and forward-thinking Registered Pharmacy Technician to join our multi-disciplinary team. The successful candidate will play a vital role in supporting the delivery of safe, effective, and efficient medication management within the surgery.

Working under the guidance of our Clinical Pharmacists and GPs, you will bridge the gap between clinical prescribing and administrative efficiency. Your primary focus will be on improving prescribing processes, undertaking medicines reconciliation, and supporting patients to get the best from their medicines.

Main duties of the job

Key Responsibilities

  • Medicines Reconciliation: Act as the first point of contact for clinical letters and discharge summaries. You will reconcile medication changes from secondary care (hospitals) and ensure the GP clinical system is updated accurately.

  • Prescription Management: Assist the prescribing team in processing repeat prescription requests, synchronising medications, and identifying where clinical reviews are required.

  • Patient Safety & Audits: Conduct targeted clinical audits (e.g., high-risk drug monitoring, MHRA alerts) to ensure patient safety and compliance with local prescribing guidelines.

  • Cost-Effective Prescribing: Lead on "medicine switches" and quality improvement projects to ensure the practice is prescribing evidence-based, cost-effective treatments.

  • Patient Liaison: Provide technical advice to patients over the phone or in person regarding their medication, such as how to use inhaler devices or explaining changes to their regimes.

  • Liaison with Community Pharmacy: Act as a key link between the surgery and local community pharmacies to resolve supply issues or dispensing queries.

Job responsibilities

As our Pharmacy Technician, you wont just be processing paperwork; you will be the clinical engine behind our prescribing safety. You will work as a key member of our Multi-Disciplinary Team (MDT), supporting our Clinical Pharmacists and GPs to ensure that every patient at Millgate receives the right medicine, at the right time, in the most efficient way possible.

Primary Responsibilities & Key Themes

1. Clinical "Transfer of Care" Specialist

  • The First Look: Be the practice lead for reconciling medications following hospital discharges or clinic letters. You will identify discrepancies, update clinical records (EMIS), and ensure that no medication "falls through the cracks" during a patient's transition from hospital to home.

  • Safety Netting: Proactively identify patients who require blood tests or clinical reviews before their next prescription is issued, keeping our patient safety standards at the highest level.

2. Medicines Optimisation & Quality (QOF/IIF)

  • Data-Driven Care: Run clinical audits to identify patients on high-risk medications (e.g., Lithium, Methotrexate, or DOACs) and ensure they are being monitored in line with national guidelines.

  • The "Green" Agenda: Assist in the practices transition to environmentally friendly prescribing, such as supporting the switch to Low Carbon Inhalers where clinically appropriate.

  • Waste Reduction: Tackle medicine wastage by promoting Electronic Repeat Dispensing (eRD) and helping patients synchronise their medications.

3. Patient-Facing Support

  • Advice Hub: Act as a point of contact for patients and community pharmacies. You will handle queries ranging from "How do I use this inhaler?" to managing medication supply issues and shortages.

  • Education: Empower patients to take ownership of their health by explaining medication changes clearly and empathetically, either over the phone or in face-to-face consultations.

4. Practice Efficiency & Innovation

  • System Design: Help us refine our repeat prescribing protocols. As a training practice, we love new ideasyoull have a seat at the table to suggest how we can use technology (like Accurx or EMIS tasks) to make our workflows smoother.

  • MDT Collaboration: Participate in clinical meetings and providing a technicians perspective on prescribing trends and challenges.
