Trainee Advanced Clinical Practitioner

Job summary

To deliver a high standard of patient care as an Advanced Practitioner in General Practice, using advanced autonomous clinical skills, and a broad and in-depth theoretical knowledge base.

This will be enabled by attending a suitable three year course at Masters Level (University of Cumbria) with supervision provided at Lancaster Medical Practice.

The course includes the Non-Medical Prescribing module.

The suitable applicant must have 3 years post registration experience and hold an honours degree at 2.2 or above.

The suitable applicant will work full time as an ACP trainee alongside mentor/supervisors. There will be full supervision and training in all aspects of the ACP role. There will be one days study leave per week to facilitate the training. There is an expectation that despite this being a training role the post holder will be a full member of the Urgent Care team and will be expected to commit to the organisation for at least two years following successful qualification.

The post-holder will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

Main duties of the job

Undertake telephone triage in order to provide total triage during surgery opening hours. This will involve providing triage cover for annual leave. Triage, treat and/or refer/direct patients presenting as same day urgent to the most appropriate service.

Recommend, instigate and explain appropriate invasive and non-invasive diagnostic tests, investigations and interpret findings and reports.

Make direct referrals to primary, secondary and social services within locally agreed pathways, guidance and protocols. Provide clinical advice to reception staff when required.

If an independent prescriber, supply and administer medicines in accordance with Patient Group Directives, prescribing safe, effective and appropriate medication as defined by current legislative frameworks.

Ensure complete and accurate documentation for each and every patient contact.

Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery. Contribute to high level care for patients with chronic diseases including diabetes; respiratory and cardiac conditions.
