Job summary

Health Navigation involves daily face to face and telephone contact with patients, arranging of appointments and follow up of patient related tasks.

The aim of the role is to support and navigate patients to the most appropriate care for their needs. It involves nonclinical triage of patients, ensuring they see the right person at the right time, but that is not necessarily a GP. This takes time and patience but is a really rewarding role, and although challenging at times job satisfaction is something that is clearly seen within our team.

A split role (navigation /coding) means you are supporting two teams as you will be working in our reception hub as well as our admin office. This will give you variety and enable you to further develop along the way.

Main duties of the job

Health Navigation involves daily face to face and telephone contact with patients, arranging of appointments and follow up of patient related tasks.

The aim of the role is to support and navigate patients to the most appropriate care for their needs. It involves nonclinical triage of patients, ensuring they see the right person at the right time, but that is not necessarily a GP.

You will ensure there is a safe and effective management of patient enquiries and patient related admin. This will include signposting and navigating to other services and liaising with patients on behalf of the clinical team.

We take pride in offering routine and non-urgent appointments either face to face or on the telephone. Patients are offered a choice, and we aim for continuity of care. We do not operate a duty doctor or triage list, therefore whoever joins our team will be doing a challenging but varied role and personality is key to our friendly team. We will offer training for the right person, even if they have no prior experience.

Coding of letters from other agencies is a very important role to ensure the patient record is updated with key clinical information. It links with navigation as you will get to know the patients and acquire a lot of medical terminology along the way.

About us

We have 10 GPs (4 partners) 4 nurses, 2 HCA's, 3 clinical pharmacists, 2 pharmacy technicians, 3 prescription clerks, 2 dietitians, a careco-ordinator and a well being coach. Our building houses the community nursing team, health visitors, midwives, podiatry, Physiotherapist, speech and language, bladder and bowel service, eye screening, urology, DVT clinic, FGM clinic, Bristol drugs project and others. We are a really busy practice of 16000 patients within Bristol Inner city. Our patient population is diverse. Anyone joining our team will benefit from the support and training we give, but also find that we do not operate on a hierarchical basis- meaning that no member of the team is any more or less important than anyone else. We are all very important cogs in a very efficient machine! We listen to each other, go out socially, and management operates an open door policy. We all support each other.
