Wyre Integrated Network PCN are recruiting for a Clinical pharmacist to join their team. The successful candidate will join a team consisting of pharmacists, pharmacy technicians, Paramedics, Social prescribers, Care coordinators and a Mental Health practitioner supported by a team of GPs all striving to support the needs of the local community.

We are looking for an energetic and motivated individual to join our innovative pharmacy team.

Main duties of the job

  • The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management
  • Provide telephone or face to face structured medication reviews
  • Manage long term conditions
  • Management of medicines on transfer of care and systems for safer prescribing
  • Manage repeat prescription authorisations and re-authorisation
  • Acute prescription requests, while addressing both the public health and social care needs of patients in the GP practice(s) that make up the PCN
  • The post holder will provide leadership on quality improvement and clinical audit and well as managing some aspects of the Quality and Outcomes Framework

The role will be supervised by our senior clinical pharmacist and work in collaboration with other members of the wider multidisciplinary team. The current PCN Pharmacy team consists of clinical pharmacist, pharmacy technicians and a senior clinical pharmacist.

The role of the PCN Clinical Pharmacist is essential for improving the quality of healthcare we provide to our patients, the successful candidate will undertake medication reviews to manage patients with complex needs and gain responsibility for areas of chronic disease management within the practices.

The role can be flexible and may be expanded in accordance with network requirements.

Qualifications

Essential

  • Registered with General Pharmaceutical Council.
  • Successful completion of a Great Britain or Northern Ireland accredited MPharm degree.
  • Evidence of commitment to CPD (continuing professional development).

Desirable

  • Previous experience of working as a pharmacist in primary care.
  • Independent prescriber or working towards/intent of gaining independent prescribing qualification.

Experience

Essential

  • Good all-round clinical knowledge and practical skills.
  • Ability to plan, manage, monitor, advise and review medication for patients in core areas, including long term conditions or a willingness to work towards this skill.
  • Demonstrates the ability to communicate complex and sensitive information in an understandable form to a variety of audiences.
  • Ability to make judgements involving complex facts or situations that require the analysis and interpretation of a range of options.
  • Experience of effective team working and ability to accept guidance as part of personal development.
  • Ability to work effectively, autonomously and be able to proactively use initiative within your own recognised professional limitations.
  • Demonstrates accountability for delivering professional expertise and direct service provision.

  • Post-qualification primary care experience.
  • Clinical examination skills.
  • Vaccination administration accredited.
