The Paramedic / Nurse Practitioner will work as part of the
multidisciplinary team working collaboratively to develop and implement
effective patient management plans.
Main duties of the job
s a Paramedic or Nurse Practitioner, you will be an integral part of our multidisciplinary team, working collaboratively to develop and implement effective patient management plans. Working closely with local community nurses and our expanding integrated care teams, you will deliver patient care in a clinical setting and, when necessary, in patient's homes. This is an exciting opportunity to join us as we work to enhance our triage process and improve access to care, where experience in clinical triage and telephone consultations will be essential for success.
In this role, you will manage daily patient triage, run your own face-face clinics, and handle referrals, prescriptions, test results, while offering support to colleagues when needed. Flexibility, a strong work ethic, and a readiness to step in where required are crucial. Ideally, you will have experience treating patients of all ages, from children to adults, and managing a wide range of medical conditions.

and pathologies in line with local and national guidelines. The post holder is
an experienced Paramedic /Nurse Practitioner
who, acting within their
professional boundaries, will provide care for the presenting patient from
initial history taking right through to diagnosis, treatment, and evaluation of
care. The post holder will be under the supervision of a senior clinician, providing
a range of services, such as triage, telephone and face-to-face appointments,
clinical decision-making, referring patients appropriately and interpreting
results. Asking advice or learning is very much encouraged and career
development is supported.

Main duties of the job
- As a part of the triage team, you will be responsible for assessing cases, prioritizing appointments according to clinical urgency, directing patients to the most appropriate pathway.
- To practice as an emergency care practitioner and demonstrate advanced clinical knowledge above those associated with conventional nursing roles.
- Assess, plan, treat and refer patients, carrying out consultations in the surgery or the community as an autonomous practitioner.
- Using own clinical judgement to diagnose and treat patients either face to face consultation, clinical telephone triage and other mediums.
- Making appropriate referrals to primary, secondary and social services care, following practice (and/or locally agreed) pathways and protocols.
- Deliver quality clinical care in a timely manner to patients.
- Supply, administer and prescribe medicines as indicated (and appropriate) to address patient need, according to medicines management policy, Patient Group Directions NMC standards or referring to registered GP as necessary.
- To recognise and initiate where appropriate relevant diagnostic investigations, using skills in this area to take the correct course of action.
- Understanding safeguarding referral process and be aware of appropriate referral mechanisms for children and vulnerable adults.
- Documentation of every patient contact and advice given. Records to be accurate in accordance with the NMC guidelines.
- Minor illness assessment.
- Be involved with continuity of care of our patients
- Prioritise own workload and ensure effective time-management strategies are embedded within the culture of the team.
- Ensure accurate notes of all consultations and treatments are recorded adequately on the computer.
- Assist in the formulation of practice philosophy, strategy and policy.
