Job summary
West Timperley Medical Centre is recruiting a Practice Nurse to join our multi-disciplinary team.
The role will be between 25 and 37 hours, the days and clinic pattern can be negotiated to best suit you and the practice.
The Practice Nurse responsibilities to include chronic disease management, smear taking, immunisations, travel advice/vaccinations and collaboration with QoF. Dedicated admin time is included in your schedule.
Main duties of the job
The Practice Nurse will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.
About us
We are a partnership practice with 4 GP Partners with 2 Salaried GPs. The practice has a wide and varied clinical team including Nurses, Student Nurse Associates, Clinical Pharmacists, First Contact Physiotherapists, a Mental Health Nurse and Social Prescribers.
We offer a range of services at the Practice including minor surgery and contraceptive fittings.
We are a teaching practice and research active.
We have a CQC rating of "Good" (2022) and good QOF achievement.

- Practice independently in accordance with NMC code of conduct and with reference to practice guidelines
- Maintain accurate documentation of care and utilisation of IT
- Ensure continuation of own personal and professional development, facilitated by utilisation of clinical supervision networks.
- Administer adult and child immunisations and vaccinations in accordance with national and local programmes
- Takes a lead in areas such as Asthma and COPD management
- Management of chronic diseases which includes asthma, hypertension, COPD, CVA/TIA, CHD, and CHD risk assessment and management. Initiates the process of diagnosis with patients suspected to have chronic disease
- Maintain disease registers
- Initiates and carries our programmes of Health promotion and screening, cervical and sexual health screening.
