Job summary

We are seeking an experienced and motivated Senior Nurse Practitioner to join our new Primary Care Network Enhanced Care & Frailty Team.

This role is central to the delivery of the EHCH PCN DES, supporting proactive, planned, and responsive care for care home residents and frail patients living in the community. The post-holder will undertake regular care home ward rounds, contribute to MDT working, and provide community home visits to support admission avoidance and continuity of care.

Main duties of the job

The role works under the clinical leadership and governance of an Advanced Clinical Practitioner (ACP) and GPs, practising with a high level of autonomy within an agreed scope. The post-holder will use advanced clinical assessment skills, manage acute and long-term conditions, and prescribe independently in line with their V300 qualification.

Dover Town PCN is aligned with three practices within Dover, providing health care to our patient population of almost 40000 people.

Our three practices are:

High Street Surgery 100 High St Dover CT16 1EQ. High Street Surgery also have a branch site in Whitfield which is a dispensing surgery.

Peter Street Surgery 108 Peter St Dover CT16 1EF. Comprises of three surgeries. Buckland Medical Centre Brookfield Place, Dover and Tara Surgery The Droveway, St Margarets which is a dispensing surgery.

St James Surgery 2 Harold St Dover CT16 1SF. Is one of 32 practices within Invicta Health across East Kent & Sussex.

Now is an exciting time to join our new Enhanced Care & Frailty Team, as we move at pace to expand our services to meet our patient population needs. The EC&F Team will be providing multi site clinics along with home visits within the community & aligned care homes to support our most vulnerable patients when they need it most.

Job responsibilities

Deliver high-quality clinical care to residents in care homes in line with the EHCH DES, including regular scheduled care home rounds.

Undertake comprehensive clinical assessments, diagnosis, and management of patients with frailty, multi-morbidity, and long-term conditions.

Provide planned and urgent community home visits to support early intervention and prevent unnecessary hospital admissions.

Prescribe independently within scope of practice (V300), escalating complex or high-risk cases to the ACP Lead or GP as appropriate.

Contribute to MDT meetings, case reviews, and care planning with PCN, community, and care home colleagues.

Support personalised care planning, including Advance Care Planning, Treatment Escalation Plans, and end-of-life care.

Maintain accurate and timely clinical records in line with professional and organisational standards.

Participate in clinical governance, audit, and quality improvement activities aligned to EHCH and frailty pathways.

Post holder must be available to work weekends on a rota basis and be committed to working across multi sites and within community completing home visits and regular visits to our aligned care homes.

Qualifications

Essential

  • Registered Nurse with current NMC registration.
  • Independent Prescriber (V300).

Desirable

  • Postgraduate study at Level 6 or above.
  • Working towards or interested in developing towards Advanced Clinical Practice.

Experience

  • Significant experience in community nursing, frailty services, care homes, or primary care.
  • Experience working in an acute clinical setting, demonstrating strong assessment and escalation skills.
  • Evidence of autonomous clinical practice within an agreed scope.
  • Ability to work effectively within a multidisciplinary PCN environment.
  • Demonstrates a compassionate, person-centred approach to care.
  • Committed to improving outcomes for frail and vulnerable populations.
  • Works collaboratively and communicates effectively with patients, families, and professionals.
  • Demonstrates accountability, professionalism, and a commitment to continuous improvement.
