Job summary

We are looking for an experienced Paramedic Practitioner to join our new Enhanced Care & Frailty Team at Dover Town PCN. This is a multi-site role delivering high-quality, person-centred care through home visits and care home support. You will play a key role in proactive care, working closely with our multidisciplinary team to improve outcomes and independence for patients across our PCN area.

Main duties of the job

This is a multi-site role, working across practices and within community visiting patients homes and aligned care homes to deliver high-quality, person-centred care. You will play a vital role in supporting patients closer to home assessing, treating, and managing health needs while helping to reduce avoidable hospital admissions.

The role includes a key focus on delivering the Enhanced Health in Care Homes (EHCH) service across 13 care and nursing homes within the PCN, working collaboratively with residents, families, and care staff to promote wellbeing and independence.

About us

Dover Town PCN is aligned with three practices within Dover, providing health care to our patient population of almost 40000 people.

Our three practices are:

High Street Surgery 100 High St Dover CT16 1EQ. High Street Surgery also have a branch site in Whitfield (Whitfield Surgery), that dispense to certain patients within the village.

Peter Street Surgery 108 Peter St Dover CT16 1EF. Comprises of three surgeries. Buckland Medical Centre Brookfield Place, Dover and Tara Surgery The Droveway, St Margarets which is a dispensing surgery.

St James Surgery 2 Harold St Dover CT16 1SF. Is one of 32 practices within Invicta Health across East Kent & Sussex.

Now is an exciting time to join our new Enhanced Care & Frailty Team, as we move at pace to expand our services to meet our patient population needs. The EC&F Team will be providing multi site clinics along with home visits within the community & aligned care homes to support our most vulnerable patients when they need it most.

Job responsibilities

Main duties of the job

Undertaking both pre-arranged and same day visits to housebound community based patients and to those living in care settings. The role requires the practitioner to be able to provide clinical assessment, diagnosis and treatment of our patients within their scope of practice as well as liaise with the patients' GP practice.

Conduct regular visits to care and nursing homes, delivering proactive and responsive care in line with the EHCH framework. Completing initial reviews with 7 working days for new residents and hospital discharges- this includes a full assessment, treatment escalation plan, personalised care planning, and DNACPR-RESPECT, where appropriate.

Use sound clinical judgment, prioritise caseloads, and refer for investigations as required.

Use sensitive, person-centred communication styles to ensure patient understanding and consent.

Prioritise and manage your workload independently while promoting high-quality care.

Promote self-care, patient education, and preventative approaches to health and wellbeing.

Work closely with GPs, nurses, pharmacists, social prescribers, and liaise with the wider PCN team, community services, SECAmb, KCHFT, Frailty Teams, Hospital and social care to deliver integrated, multi-disciplinary care.

Participate in weekly Multi-Disciplinary Team meetings to support care planning for complex or vulnerable patients

Support proactive management of long-term conditions, frailty, and palliative care needs.

Evaluate treatment plans and offer advice to care home staff. Effectively use EMIS and other clinical systems to plan care, record interventions, and support audit processes.

Maintain professional registration and engage in ongoing CPD and supervision.

Support a learning culture by sharing knowledge and contributing to team development.

Participate in training, reflective practice, and relevant courses or conferences.

Promote equality and diversity in all interactions.

Respect the privacy, dignity, and beliefs of patients, families, and colleagues.

Engage in annual performance reviews and maintain a professional development portfolio.

Support innovation and help develop new models of care in collaboration with colleagues and line managers

Undertake tasks aligned with the scope and responsibilities of the role.

Duties may be subject to change in response to service needs, with agreement from the practice or PCN.

Post holder must be available to work weekends on a rota basis and be committed to working across multi sites and within community completing home visits and regular visits to our aligned care homes.

Qualifications

Essential

  • BSc in a training programme approved by the College of Paramedics.
  • Active and relevant professional registration with HCPC
  • Completed two-year Consolidation of Learning period as a "newly qualified paramedic" AND have a further three years' experience as a band 6 (or equivalent) paramedic
  • Commit to obtaining the clinical pillar competencies of the paramedic FCP/AP roadmap to practice within six months of the commencement of the role

Desirable

  • Evidence of verification against the paramedic primary care roadmap
  • Independent or supplementary prescribing qualification, or willingness to undertake

Experience

Essential

  • Experience in managing the care of patients presenting with undifferentiated undiagnosed conditions and a designated caseload, using theoretical and practical experience, completing examinations and assessments, care planning, implementing interventions, referring appropriately, and evaluating outcomes aligned to care plans
