Job summary

We are seeking a dedicated Practice Nurse to complement our healthcare team. The Practice Nurse will play a crucial role in providing high-quality patient care within our practice. The candidate will have a background in Practice or Community nursing.

Main duties of the job

Main duties to include :

  • Administer both Childhood and Adult vaccinations (including Flu, Shingles, Pneumococcal etc)
  • Provide Chronic Disease Management (Asthma, Diabetes, COPD etc)
  • Perform Cervical Cytology Screening
  • Conduct patient assessments and provide General Practice Nursing care in accordance with General Practice policies
  • Educate patients on health management and preventive care strategies
  • Maintain accurate patient records and ensure confidentiality
  • Collaborate with interdisciplinary team members to optimise patient outcomes
  • Lead on Infection Prevention & Control with support from other Health Care staff

About us

Clayton Brook Surgery have an excellent cohesive and dedicated team including; 2 GP Partners, Salaried GPs, Business Manager, Practice Manager, Operations Manager and a strong administrative team. Our GP Partners are trainers with an emphasis on training GPs of the future.

The permanent role will be a Part-time post of 24 hours

The post holder will be flexible in their approach and work closely in collaboration with staff from Moss Side Medical Centre in Leyland.

Our purpose is to provide high quality personal healthcare for people registered with the Practice and to seek continuous improvement in the health status of the overall practice population.

We aim to achieve this by developing and maintaining a happy effective Practice that is responsive to people's needs and expectations and which reflects whenever possible the latest attention in primary care.

  • Maintain high standards of care within NMC professional Code of Conduct
  • Maintain own clinical/professional expertise by participating in clinical activities.

Clinical Duties to include:

  • Lead on agreed clinical areas in relation to QOF, LQC & other Enhanced Services
  • Responsible for management of treatment and continuing care for patients on an open access basis.
  • Overall responsibility for nursing management of groups of patients, for example, diabetes, asthma, hypertension, including responsibility for developing, implementing and evaluating programmes of care as per practice protocols.
  • Provide health promotion advice where appropriate with referral to GP as appropriate.
  • Administer immunisations of children and adults, for example tetanus boosters, holiday vaccinations, and influenza, pneumococcus.
  • Provide cervical smear taking in the practice.
  • Support with family planning advice where appropriate and within competence
  • Provide health assessment services
  • Assist in the teaching of other nurses, GP registrars and HCAs etc.
  • Will be involved in selecting, carrying out and interpreting specific tests.

Administration:

  • Enter all clinical consultations into clinical system
  • Utilise all clinical templates contained in clinical system in line with Practice guidance on best practice

Management:

  • Exercise leadership and aim to achieve high morale within the nursing team by efficient management skills, good staff relationships and good clear lines of communication.
  • Give support to junior staff.
  • Work in liaison with facilitators in identifying individual educational development needs.
  • Ensure staff awareness of and conformity with nursing policies and procedures.
  • Provide nursing input to any decision making process affecting patient care.
  • Oversee the monitoring, stock control and documentation of drugs, vaccinations, immunisations and clinical stores according to practice and legal requirements.

Leadership Role

  • Apply national, local and practice policies, standards and guidance
  • Ensure nursing team aware how applying local and practice policies, standards and guidance affect own work
  • Participate in audit where appropriate
  • Lead in the development and delivery of specialist primary care services e.g Long Term Disease Management
  • Ensure National Service Frameworks (e.g CHD, Diabetes, Mental Health) are being delivered
  • Lead on Clinical Infection Control working with non-clinical infection control lead to meet the standards required of the practice. Including monthly and annual audits and training of staff (including waste, sharps and hand hygiene, clinical hazards)

Maintenance of Treatment and Examination Rooms:

  • Lead on ensuring all equipment is clean, safe and organise sterile supply.

  • Lead on cold chain and daily vaccine fridge checks
  • Lead on ensuring clinical rooms are complaint with IPC and CQC standards
