Main duties of the job

Qualifications: Nurse Practitioner degree (or working towards qualification)

Extended Nurse Prescriber (or working towards qualification)

Experience: Experience within a similar role is essential (2 Years min)

Knowledge/skills: Keyboard skills

Job responsibilities

The practice is looking to add to its existing team of Nurse Practitioners. The posts will help improve access for patients and help to increase our capacity to deal with on-the-day demand for appointments. The role of the nurse practitioner will be to see and treat mainly acute patients. As a new role there will be scope for the successful candidate to help to develop and evolve this position within the practice.

The main aim objectives and responsibilities will be:

Making professionally autonomous decisions

Receiving patients with undifferentiated and undiagnosed problems.

Making differential diagnosis using decision making and problem solving skills.

Provide comprehensive healthcare sensitive to patients needs, including assessment, diagnosing, prescribing, and referrals where appropriate. Complementing the roles of GPs and the multidisciplinary team.

Conducting physical assessments to diagnose patients conditions and provide appropriate self-care advice, prescribing treatment, or referral using evidence based guidelines.

Direct and participate in provision of continuing care, health maintenance, screening and health promotion to patients as part of patient centred multidisciplinary team.

Managing and participating in Chronic disease management

Supporting and developing work towards agreed GMS targets.

Maintain clinical knowledge and skills and identify training needs.

Any other duties as may be required by the practice in providing quality health care to patients.
