  • Ashfield and Manston Surgery are recruiting for 3 Care Navigator positions to work across to join our expanding practices. The partnership has recently taken over Manston surgery to now run both practices. Our experienced leadership team is helping to deliver modern general practice taking advantage of at scale working while also maintaining consistent patient care from teams that know patients well.

Main duties of the job

Provide general assistance to the practice team and project a positive and friendly image to patients and other visitors, either in person or via the telephone

Job responsibilities:

Reception Duties

Navigate patients to the most relevant treatment provider.

Utilise appointment system and monitor flow of patients into consulting and treatment rooms.

Assist patients with understanding practice operating procedures.

Advise patients of relevant charges for private services, accept payment and record in line with agreed protocol.

Respond to all queries and requests for assistance from patients and visitors.

Enter requests for home visits onto the computer following practice protocols.

Ensure prescriptions are actioned via all channels following practice protocols.

Action requests for ambulances following practice protocols.

Ensure reception and waiting areas are kept neat and tidy.

Adhere to correct opening and closing procedures for the practice.

Job responsibilities

Job Responsibilities:

Helping patients to navigate NHS and local services by providing information

Signposting and triaging patients following the patient submitting a request via our Online Consultation Portal

Managing prescriptions requests in accordance with Practice policies

Booking patient appointments with clinicians and home visits

Supporting clinicians during surgeries including acting as a patient chaperone (Training will be given)

Giving out test results,

Liaising with other primary and secondary care providers

Accurately scanning of hospital letters and other clinical correspondence onto the clinical system

Keeping patient records up-to-date and recording accurate information as appropriate including read coding

Handling samples in accordance with practice health and safety protocols and undertaking urine dip stick testing (full training will be provided);

Patient chaperone (training will be given)

Preparing the consulting rooms prior to surgery and tidying up the rooms after surgery, check stock levels of stationary and medical supplies

General housekeeping eg keeping reception and waiting room tidy

Allocated administrative tasks such as Tasks on the clinical system, copying medical notes, handling incoming emails etc

Handling money for private non NHS work such as medical reports

Attending training events as required and monthly target meetings

Any other duties as directed by the Management team

  • * Competent use of IT
  • * Ability to deal confidently and courteously with people both in person and over the telephone.

  • * Able to work on own initiative and prioritise workload
  • * Empathetic, honest, caring
  • * Diplomatic and considerate
  • * Able to work under pressure
  • * Willing to work flexible hours as necessary
  • * An understanding, acceptance and adherence to the need for strict confidentiality
  • * To have had first two covid vaccines
  • * Have a yearly flu vaccination
  • * Have hep B vaccination course
