Job summary
This is a fantastic opportunity to apply
for an innovative new role. We are seeking a Health and Wellbeing
Coach to consolidate the partnership between Platt Hall and Better Health MCR.
The post holder will build on an established foundation of health work at
Platt Hall and will be embedded equally, working in both organisations. They will work at both sites to reduce health inequalities using Platt
Hall's programmes, collections and building as a key resource. The
post holder will take a lead in supporting people to self-identify issues and
encourage proactive prevention of new and existing illnesses, with a focus on
using evidence-based strategies.
Main duties of the job
The Health and Wellbeing Coach will take an
approach that considers the whole person in addressing existing issues, working
alongside people to coach and motivate them through multiple sessions,
supporting them to identify their needs, set goals and help them to implement
their personal health and care plan. They
will develop wellbeing groups at Platt Hall in partnership with patients. They
will take an approach that is non-judgemental, that supports personal choice
and positive risk-taking, that addresses potential consequences, and ensures
patients understand the accountability of their own decisions. The post holder
will develop and maintain positive relationships with key stakeholders,
providers and local communities.
You will support patients to
implement their personalised health and care plan using the Platt Hall
building, collections and programme as a resource.
This post currently has funding for
two and a half years which may be increased up to three years.

work. The Platt Hall team currently work in partnership with Better Health MCR
to develop and recruit for a range of wellbeing programmes in the Hall, garden
and park. Platt Hall is now expanding the team, to bring a different kind of
expertise into the organisation and better reflect the diverse communities of
the neighbourhood.

be part of the NHS Pension Scheme, with an employer contribution of 20.6%. We
all need a well-deserved break: annual leave goes up to 6.6 weeks holiday plus Bank Holidays (pro rata), depending on length of NHS service.
