Job summary

Limes Medical Centre are looking for a friendly, driven and motivated individual to join our team.

Previous experience in minor illness and triage in a primary care experience is essential. You must be able to demonstrate safe clinical care to our patients acting within your professional boundaries.

All applicants must have completed their non-medical prescribing and be confident in working autonomously in primary care under the guidance of our GPs.

The successful post holder will be experienced in working in a primary care settings as a nurse practitioner, providing high quality care to our community. From initial history taking, assessment to diagnosis and treatment. They will demonstrate safe, clinical decision-making and expert care.

They will work collaboratively with the Limes Medical Centre team to meet the needs of our patients, supporting the delivery of policy and procedures.

**The post is over two sites, the main site at Limes Avenue, Alfreton and our satellite site - Blackwell Medical Centre

Main duties of the job

• NMC Registration Non-Medical Prescribing.
• Treatment of minor illness.
• Assessment, diagnosis, treatment, precribing, referral and discharge as appropriate.
• Manage long term conditions.
• Work autonomously.
• Support QOF outcomes.

Limes Medical Centre is a friendly practice, offering a supporting working environment. We are a three partner training practice, with three salaried GPs, three practice nurses, two HCAs and a phlebotomist.

We are an active member of South Hardwick PCN supported by first contact physios, pharmacists, social prescribers and a mental health practitioner.
