Job summary
Foundry Practice is looking to appoint a Clinical Pharmacist to work as part of a small and very supportive multi-disciplinary team.
You will be a key member of the Practice working directly with patients in optimising their medications as part of their long term conditions management as well as providing support to the practice staff in matters relating to prescription and medication enquiries.
It is expected that the postholder will be on the CPPE pathway, and working towards (or already be) a non-medical prescriber.
Main duties of the job
- Undertake clinical structured medication reviews with patients and produce recommendations for clinical pharmacist, nurses and/or GP on prescribing and monitoring.
- Be available to offer advice and support to the Prescription Admin Team dealing with patientsquestions, queries and concerns about their medicines.
Answers relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams (e.g. community pharmacy) and patients with queries about medicines.
Work with the Senior Management and Clinical team to ensure the practice is compliant with CQC standards where medicines are involved.
Provide education and training to primary healthcare team on therapeutics and medicines optimisation.
Support the delivery of QOF, LCS, and medicine optimisation programmes by working collaboratively with other Foundry team members and the ICB.
