Job description
Job responsibilities
Seeing
15 patients @ 10-minute consultations per patient with associated admin and
paperwork.
a.
On
Call duties including actioning blood tests daily.
b.
Home
visits as allocated by the on-call GP on routine days of work
c.
16 allocated tasks per full day of routine
work, or 8 on a half routine day
d.
Signing of ETP prescriptions during on call
sessions, and other routine sessions to support on call GP.
e.
Filing personal pathology links on all routine
days of work.
f.
On call sessions requires filing of ALL
pathology links (clinicians at work that day can file their own as above)
that are received after 6pm the previous day and up to 1pm the following day
when doing morning on call. Bloods
from 1pm until feasible (5.30pm ideally) when doing an afternoon on
call. On call sessions also require
additional urgent tasks to be actioned in addition to the above quotient, E.G
UTI queries, queries from nursing team, community requests, palliative care
medicine requests etc.
The following are the core
responsibilities of the nurse practitioner. There may be on occasion, a
requirement to carry out other tasks; this will be dependent upon factors
such as workload and staffing levels:

Develop, implement and embed health promotion and
well-being programmes
h.
Provide a triage service, actively managing or
referring patients or treating patients within own scope of practice
i.
Support the clinical team in the development of
individual treatment plans for chronic disease patients
j.
Identify, manage and support patients as risk of
developing long-term conditions, preventing adverse effects on the
patients health
k.
Request pathology services as necessary, processing and
interpreting results as required
l.
Refer patients directly to other services as
appropriate
m.
Provide chronic disease clinics, delivering patient
care as necessary, referring patients to secondary / specialist care as
required
n.
Maintain accurate clinical records in conjunction with
extant legislation
o.
Ensure read codes are used effectively
p.
Maintain chronic disease registers
q.
Prioritise health issues and intervene appropriately
r.
Support the team in dealing with clinical emergencies
s.
Recognise, assess and refer patients presenting with
mental health needs
t.
Recognise, assess and refer patients with community
health needs, developing strategies for effective care pathways
u.
Liaise with external services / agencies to ensure the
patient is supported appropriately (vulnerable patients etc.)
v.
Delegate clinical responsibilities appropriately
(ensuring safe practice and the task is within the scope of practice of the
individual)
w.
Support the clinical team with all safeguarding
matters, in accordance with local and national policies
x.
Understand practice and local policies for substance
abuse and addictive behaviour, referring patients appropriately
y.
Deliver opportunistic health promotion where
appropriate
z.
Complete
