Job summary

NTGP has a fantastic opportunity for a Clinical Pharmacist to join our multi-disciplinary team. The position will require sessions from both locations (on different days). We are flexible with sessions available. The days can be negotiated to suit you and the requirements of the practice.

The Clinical Pharmacist will act autonomously within their professional scope of practice providing care for patients face to face, telephone consultations and home visits.

You will be responsible for the provision of medical services to our patient population managing a caseload dealing with a wide range of health needs. You will be supporting & supported by the wide range of clinical and non-clinical team within the practice

Main duties of the job

The post holder will manage and undertake the following duties:

  • Conduct medication reviews (face-to-face, telephone, and via digital platforms) for patients with polypharmacy, long-term conditions, or complex needs.
  • Acute and minor aliment clinics within the candidates competency framework
  • Perform physical examinations, interpret diagnostic tests, and prescribe medications as necessary
  • Deal with incoming hospital discharges and third party letters
  • Support repeat prescription processes and medicine reconciliation post-discharge.
  • Educate patients and their families about health conditions, treatment options, and preventive measures.
  • Participate in quality improvement initiatives and contribute to the development of clinical guidelines.
  • Stay current with advancements in medical practices and participate in ongoing professional development.
  • Promote antimicrobial stewardship and best practice prescribing

About us

We are large forward-thinking practice of 15,000 patients, providing quality healthcare and outcomes for our patients in the North Trafford area. We operate from two sites Chester Road and Seymour Grove.

The team at NTGP comprises of 3 GP Partners, 8 Salaried GPs, 2 Physician Associates, 2 Pharmacists, and 6 Nursing team along with non-clinical team of 30+ staff managed by the Business Manager.

Our practices have been rated as Good by CQC and we take pride in delivering quality care to our patients and have a strong training and personal development ethos. We are part of North Trafford Primary Care Network one of the partners is the Clinical Director.

We are using EMIS Web and DOCMAN as our clinical system along with Ask My GP to provide digital platform to our patients.

Person Specification

Qualifications

Essential

  • GPhC registration as a qualified Pharmacist.
  • Strong clinical knowledge with experience in primary care, hospital pharmacy, or community setting.
  • Independent Prescribing qualification (or working towards).
  • Excellent communication, consultation, and organisational skills.
  • Ability to work independently and collaboratively within a multidisciplinary team.
  • Strong commitment to continuous professional development

Desirable

  • Experience working in a GP surgery, PCN, or other primary care environment.
  • Familiarity with EMIS Web.
  • Experience with QOF, IIF, or medicines audits.
  • Additional qualifications in clinical pharmacy, long-term condition management, or patient education.
