About the Role
Were looking for a proactive and organised Care Coordinator to join our friendly team at Park Surgery. You'll support both patients and staff to improve care coordination, assist service performance (QOF, IIF, LCS, etc.), and assist management to ensure the practice runs smoothly day-to-day. This is a hands-on role, ideal for someone who thrives in a busy healthcare environment and is passionate about patient care and working in a larger team.
Main duties of the job
Key Responsibilities
- Coordinate patient care and support uptake of enhanced services and incentive schemes
- Assist Practice Managers in project delivery, audits, and practice development
- Collaborate with admin, reception, and nursing teams to support with staffing and workflow issues
- Maintain appointment schedules and clinical rotas
- Support with data quality, coding, and reporting requirements
- Handle internal and external communications professionally
- Contribute to compliance processes, HR admin, and new staff onboarding
- Help maintain training compliance, annual leave , and staff files
- Support the practice with promoting services via the website, leaflets, PPG, and patient comms
- Attend and minute meetings
About us
Our surgery Consists of:
Non-Clinical - 2x Practice Managers, 4x Medical Secretaries, 9x Receptionists and 2x care coordinators
Clinical - 9x GPs, 3x Practice Nurses, 1x HCA, 1x NA and 1x Phlebotomist
We also have access to a wide range of PCN Staff - including Physiotherapist, Mental health practitioners, Pharmacists, Pharmacy Technicians, Social Prescribers and Care Coordinators.
