A vacancy for an Advanced Clinical Practitioner
has arisen within our busy and friendly clinical team at Filey Surgery. We are looking to recruit a proactive, energetic and
positive ACP to join our enthusiastic team. You will be fully qualified
to provide a range of services such as assessment, diagnosis, treatment, clinical
triage & referring patients, have a demonstrable ability to work within a
multidisciplinary team. You would be required to work autonomously in the
practice & provide home visits.
You will be a caring
and dedicated professional who is flexible and able to work independently under
minimal supervision. The post holder will
need to have a willingness to access training and maintain registration at all
times.
This vacancy is for 27-28.5
hours per week, (over 3 days). Training and development will be available and offered to a
candidate who shows a willingness and commitment to the post. A trainee ACP or
individual wishing to train may be considered.
All our
Practice Team members support the extended access rota and as such you will be
required on occasion to work evening or weekend shifts.
Main duties of the job
To be responsible for the provision of routine and acute medical
services, working as part of the practice multidisciplinary team, delivering
care within their scope of practice to the entitled patient population. You
will provide a range of services such as assessment, diagnosis, treatment,
telephone triage and referring patients appropriately, as well as supporting
the management team in the reviewing of clinical policy and procedure.

Primary Responsibilities
The following are the core responsibilities of the ACP. There may be on occasion a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels. You will work within your own scope of clinical practice and:
- Provide clinical leadership and supervision to colleagues
- Provide planned or unplanned care to patients as required, in accordance with clinically based evidence and NICE guidance.
- Triage, examine, diagnose offer treatment, or liaise on patient care with the most appropriate member of the clinical team as required.
- Maintain accurate clinical records in line with legislation, ensuring read codes are used correctly.Ensure continuity of care, arranging follow-up consultations or reviews as necessary
- Undertake home visits to patients
- Review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes
- Recognise, assess and refer patients presenting with mental health needs where appropriate
- Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)
- Acting in accordance with local and National safeguarding policies and procedures and having an awareness of vulnerable patients in our community
- Liaise with external services/agencies to ensure that the patient is supported appropriately (vulnerable patients, etc.)
- Ensure that they adhere to the relevant patient group directives and local clinical pathways at all times
- Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
- Deliver opportunistic health promotion where appropriate
SecondaryResponsibilitiesIn addition to the primary responsibilities, the Advanced Care Practitioner may be requested to:
- Support the practice audit programme, undertaking audits when necessary
- Support junior members of the team, providing guidance when necessary
- Participate in local initiatives to enhance service delivery and patient care
- Support and participate in shared learning across the practice
- Continually review clinical practices, responding to National policies and initiatives where appropriate
- Participate in the review of significant and near-miss events, applying a structured approach, i.e. root cause analysis (RCA)
- Take personal responsibility for own learning and development, including the requirement to maintain currency, achieving all targets set in own Personal Development Plan (PDP)
Generic Responsibilities
All staff at Filey Surgery have a duty to perform the following:
Training and personal development
- Working at a Masters' level or equivalent that encompasses the four pillars of clinical practice, leadership and management, education and research, demonstrating core capacilities with specific clinical competencies.
- Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individuals responsibility to remain up to date with recent developments.
- Participate in the education and training of students of all disciplines and the introduction of all members of the practice staff where appropriate
- Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development.
- If it is necessary to expand the role to include additional responsibilities, full training will be given.
- Develop and maintain a Personal Learning Plan
Liaison
- There is also the need to establish and maintain good liaison with other surgeries and agencies, including secondary care and commissioners.
Meetings
- It will be necessary to attend and contribute to various practice meetings as requested. The only reason for not attending will be annual, study or sick leave.

- In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately
