Job summary

We are looking to welcome an experienced, caring and enthusiastic Practice Nurse to join our existing Nursing Team at Oakley Medical Practice. We are a modern, supportive, forward thinking Practice based on a single site in South Leeds who strive to provide high quality services and care for our patients. We are high achievers in QOF with a CQC rating of Good. The ideal candidate must be qualified to manage long term conditions including Asthma, COPD, diabetes and CHD in line with local and national guidelines and cervical cytology.

Main duties of the job

The ideal candidate must be qualified to manage long term conditions including Asthma, COPD, diabetes and CHD in line with local and national guidelines and cervical cytology.

Other responsibilities include:

  • administering medicines and treatments,

  • performing diagnostic tests,

  • promoting health and well-being and person centred plans for patients with/at risk of a long term condition ensuring patients health and social care needs are met

  • cervical cytology and promoting all screening programmes

  • vaccination and immunisation programmes for both adults and children

  • wound care

  • new patient /NHS health checks

  • assisting the clinical team as and when appropriate

A good understanding and experience of using SystmOne clinical system is desirable.

Partners / GPs / Practice Manager

Responsible for:

Job Purpose:

To provide a high quality care service based on the needs of the Practice population which includes treatment, screening, preventative care and patient education

Duties and Responsibilities

Clinical

1.

To manage patients with chronic disease, running specific clinic sessions and maintaining robust call and recall systems

2.

Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule

3.

Treat patients with minor injuries

4.

Provide directly and assist GPs with contraceptive services

5.

Break bad news to patients/parents/carers and provide appropriate ongoing support to meet identified needs

6.

Act as chaperone when required

7.

Undertake health promotion to help educate patients

8.

Select, carry out and interpret results of specific tests in line with competencies

Administration

9.

To maintain accurate patient records and enter onto the computer using agreed Read Codes

10.

Ensure accurate completion of all necessary documentation associated with patient health care

General

11.

To maintain own annual registration with the NMC and adhere to Code of Professional Conduct

12.

To maintain a safe and therapeutic environment for all PHCT staff, patients, carers and co-workers from other agencies. Ensure all statutory regulations (e.g. Health & Safety, COSHH, etc) are implemented and adhered to

13.

To promote a professional approach at all times by attire and attitude to patients and staff both in and out of the Practice

14.

To maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice

15.

Attend and participate in staff and Practice meetings and in-house clinical training sessions

16.

Any other reasonable duties that may be required from time-to-time
