Job summary

Woodhouse Health Centre is looking for an experienced Practice Nurse to join our team of Nurses and HCAs. The ideal candidate would be dynamic, able to work on their own initiative, committed, and a team player.

Main duties of the job

Your role would be to deliver effective healthcare to our patients, including long-term condition management, wound management, vaccinations, and cervical cytology, depending upon clinical competence.

You would also complete appropriate clinical coding in line with QOF and IIF targets.

About us

Woodhouse Health Centre is one the largest health centres in the area with around 12,000 patients. We cover a wide area and are an extremely busy Practice with a large amount of vulnerable and elderly patients. We are also a GP training practice.

We have six GP partner's, salaried GP's, GP Registrars and F2 Doctors. Nurses, HCA's, Pharmacists, Paramedics, and a variety of other clinical roles.

Job description

Job responsibilities

To work as part of the Practice's multidisciplinary team, the post holder will effectively deliver nursing services to the entitled patient population.

To provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.

To prescribe medication for patients in accordance with PGDs and own scope of practice.

To review patient medications to enhance compliance.

To oversee the provision of wound care services to patients.

To co-ordinate the provision of travel medicine services.

To request pathology services, as necessary.

To process pathology results, as required.

To oversee the provision of chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.

To maintain accurate clinical records in conjunction with extant legislation.

To ensure ensure effective use of read codes.

To chaperone patients where necessary.

To prioritise health issues and intervene appropriately.

To support the Team in dealing with clinical emergencies.

To recognise, assess and refer patients presenting with mental health needs.

To implement vaccination programmes for adults and children.

To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice).

To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc).

To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual).

In the first instance we would be looking at the working pattern being: Mondays 8.00am-5.30pm (including a 30 minute unpaid lunch break), and Thursdays 8.00am-1.00pm, with the potential for further hours.
