Job summary

Woodhouse Health Centre is looking for a new Advanced Nurse Practitioner/Physician Associate to join an experienced team of doctors, nurses and HCAs and a wider clinical and administration team. The ideal candidate would be dynamic, committed, and capable of maintaining positive relationships within the team. We are looking for someone to work full time, but we would consider part time, depending on the candidate.

Main duties of the job

Your role would be to deliver effective healthcare to our patients, including initial history taking, clinical assessment, diagnosis, treatment, prescribing and evaluation of care.

About us

Woodhouse Health Centre is one the largest health centres in this area with around 12000 patients. We cover a wide area and are an extremely busy Practice with a large amount of vulnerable and elderly patients. We are also a GP training practice.

We have six GP partner's, salaried GP's, GP Registrars and F2 Doctors. Nurses, HCA's, Pharmacists, Paramedics, and a wide variety of other clinical roles.

Job responsibilities

  • Work autonomously and be accountable for his/her own professional actions.
  • Undertake clinical practice at an advanced level using expert knowledge and clinical skills to deliver holistic care to people accessing primary care services.
  • Discuss with GP and, where appropriate, make referrals
  • Prescribe medication as appropriate.
  • Maintain accurate record keeping in SystmOne.

  • Request and interpret tests appropriate to patients need.
  • Reduce inappropriate demand for emergency services by referring patients to appropriate health and social care agencies.
  • Deliver evidence based clinical practice by utilising applied knowledge of clinical audit, critical appraisal of research, research methodology, ethical review and research governance.
  • Make appropriate use of clinical decision support software and guidance documents.
  • Work as an effective member of a multi-professional team. Maintain an up-to-date knowledge of clinical practices, ensuring that these are reflected in compliance with policies and procedures.
  • Communicate effectively with staff and managers on a regular basis, both individually and at team meetings.
  • Recognise peoples needs for alternative methods of communication and respond accordingly when communicating with patients and carers.
  • Assist the GPs, Managers and Partners by promoting and establishing a healthy and safe working environment for all personnel. Provide clinical advice and feedback on matters of health and safety, risk assessments, accident reporting and investigations.
