Job summary
WholeCare is seeking to appoint a Primary
Care Chaplain to provide a Chaplaincy
/ Listening Service as a specialism within the range of Talking Therapies
provided by the NHS.
The role is primarily based at Mythe Medical Practice in Tewkesbury to offer patient support to the practice located within the Devereaux Centre.
The role is for 26 hours per week, flexible over Monday to Friday
Salary - £32,073 full time equivalent. Pro rata for part time hours.
Please note this role does not qualify for entry into the NHS pension scheme.

Main duties of the job
Primary Care
Chaplains support people to take pro-active steps to improve the way they
manage their physical, mental, social and spiritual health, based on what
matters to them.
They support people to develop their knowledge, skills and
confidence, or to build their patient activation, in managing their health and
care, to improve their health outcomes and quality of life.
Primary Care
Chaplains do this by active listening and motivating patients through multiple
sessions to identify their needs, set goals, and supporting patients to achieve
their personalised health and care plan objectives and providing interventions
such as self-management, education and peer support.

The role will be based within Mythe Medical Practice, situated in the heart of Tewkesbury.
Our 9 GPs (6 partners and 3 salaried) supported by a
wider clinical team of 2 nurse practitioners, nurses, 3
HCAs. The administrative and management
team comprises of staff with varied administrative
and management experience, including nursing specialist
trained in chronic disease management, QOF recalls management team and a team
of pharmacists and pharmacy technicians.
A training practice hosting up to 3 GP registrars supporting 2 medical students from Three
Counties Medical School.
Our internal focus is on wellbeing of our staff, achieved
with learning events, staff wellbeing sessions (such as yoga
run by one of our salaried GPs), regular meetings sharing clinical knowledge
and providing support to colleagues.

Chaplains
manage and prioritise a caseload, according to the needs, priorities and
support required by individuals in the caseload. They identify when a persons
needs are beyond the scope of the Chaplaincy role and refer them back to other
health professionals or organisations. Chaplains may work with people by phone,
by video conference, or face-to-face.
The
successful candidate will be kind, reflective and self-aware and will enjoy
working with a wide range of people. They will have good communication and
negotiation skills and a firm belief that people have untapped resources within
them, that can be unleashed by providing a high quality, non-judgemental
service. They will be proactive with a flexible attitude, keen to work as part
of a team and committed to developing themselves in this skilled role and
supporting colleagues to do the same.
Chaplains
are a key part of the Primary Care Network (PCN) multidisciplinary team. They
often work alongside Social Prescribing Link Workers and Care Coordinators to
provide an all-encompassing approach to personalised care and promote and embed
the personalised care approach across the Primary Care Network(s).
Chaplains
will have access to ongoing supervision, skills development, and support so
they are able to further build their skills and experience within the role.
The service
provided by Chaplains is a specialism within the range of talking therapies.
This professional listening support service is offered within Primary Care
settings based on an approach to health and wellbeing which addresses the
emotional, psychosocial and spiritual needs of patients. This approach has been
found to be particularly effective for patients experiencing loss or change in
their lives, e.g. in relationships, work, health, bereavement, confidence,
purpose, lifestyle, self-efficacy, relocation, crisis, trauma or mental health.
This service, also known as Listening and Guidance, draws on more than 25
years experience of service provision in primary care. It is not a religious
service and is available to patients of all faiths and beliefs. The specialism offered by Chaplains therefore
will most likely benefit patients whose spiritual needs are affecting their
health and wellbeing. Spiritual needs are understood as the universal human
need for a sense of significance, identity, purpose, belonging, meaning and
hope.
