Job summary
Healthier West Wirral Primary Care Network have an exciting, new opportunity for a Care Home Care Coordinator. This is an administrative role that will specialise in delivering enhanced health services for the care home population.
We require the Care Coordinator to work 20 hours per week across Monday-Friday.
This is a fixed term contract over 12 months with a view to make permanant.
Main duties of the job
The post holder will:
Have excellent organisational and communication skills.
Be interested in working with people with frailty and aware of advanced care planning components, such as DNARs (Do Not Resuscitate) and EHCPs (Emergency Health Care Plan).
Be suitably proficient with IT systems and software applications, preferably in a clinical setting.
Exhibit safe, professional decision-making and a high level of care for patients within the Network.
Work collaboratively with the wider Network team to meet the needs of our diverse range of patients.

Key Responsibilities
The following are the core responsibilities of the Care Home Care Coordinator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
To provide administrative support for the Care Home team, ensuring an efficient and effective service is maintained. Where necessary, feedback any concerns to the service lead.
Foster and maintain effective relationships with key stakeholders including care home patients, care home teams, GP practices, clinicians, and other healthcare professionals.
To support the implementation and maintenance of the Enhanced Health in Care Homes Framework within all CQC (Care Quality Commission) registered care homes under the care of HWW PCN.
Support clinicians in delivering proactive care through weekly ward rounds, advanced care planning, and multidisciplinary team meetings.
Make appointments and book patients in for appointments across all PCN services when required.
Compiling lists of patients, managing lists, ownership and oversight of proactive care and patient contacts.
Managing booking of proactive appointments on behalf of the clinical team, ensuring that appropriate patient information has been gathered in advance.
Advising management regarding appointment utilisation and patient demand.
Support with the COVID vaccination program, care home planning and support with the admin of vaccination clinics.
Support with care home MDT (Multi-Disciplinary Team) administration as required.
Support Network Manager with the induction of new staff, allocating time into their induction plan to share care home service processes.
Being the first point of contact for matters relating to care home administration and supporting care home teams with concerns or queries relating to residents.
To provide communication between practices, clinical teams, care homes and service leads.
To work in accordance with written protocols.
