Job summary

Healthier West Wirral Primary Care Network have an exciting, new opportunity for a Services Care Coordinator. This is an administrative role that will specialize in patient bookings.

We require the Services Care Coordinator to work between 30 and 37.5 hours per week across Monday-Friday.

Main duties of the job

  • The Services Care Coordinator will be responsible for staff rotas including planning for annual leave and study time, so good organisational and interpersonal skills are essential.
  • Support the planning and coordination of all service clinics, including making patient bookings and sending patient texts.
  • setting up clinical session using Emis software.
  • Team Coordinator will be expected to carry out general administrative work and adhoc office duties from time to time to support our clinical teams.

We are an ambitious and forward thinking organisation that has already had much success building a large PCN team including Physician Associates, Phamracists, Care Coordinators and Paramedics.

Job responsibilities

KEY RESPONSIBILITIES:

  • To have a thorough knowledge of all PCN policies and procedures.
  • To work in accordance of written protocols.
  • Provide administrative support for management and clinical staff.
  • being first point of contact for matters relating to Patient bookings.
  • planning and organising call and recall of patient proactive care appointments.
  • Setting up clinics on Emis. Reviewing and amending clinics when needed. Advising management regarding appointment utilisation and patient demand.
  • Creating weekly staff rota ensuring this is effectively communicated to staff and practices.
  • Supporting workload of other Coordinators in the team when needed.
  • providing support to reception teams when required (this may result in rare occasional Saturday working)
  • monitoring data and preparing KPIs reports

  • Running searches for patient safety alerts and other medicines safety work
