Job summary
Radford Medical Practice is seeking a Care Coordinator who is passionate about supporting others and making a real difference every day. This role is ideal for someone who is proactive, highly organised and motivated.
As a Care Coordinator, you will play a key role in supporting patients in receiving personalised, coordinated care.
You will work closely with Primary Care Network (PCN) colleagues to identify and manage a caseload of patients.
We are keen to attract a caring and passionate individual to join our experienced team.
The post holder will be required to work across our two surgeries covering opening hours of 8am - 6.30pm for 37.5 hours per week.
Main duties of the job
As a patient-contact role, the post holder will be responsible for proactively identifying and supporting patients and their carers in the development of personalised plans, making appointments and in the coordination and navigation across health and care services.

We are a long standing, busy inner city surgery based across two sites.
We currently have around 20,000 patients with a large student population based at the Nottingham Trent University and a smaller diverse population based at our inner city Radford site.
