Job summary

We're looking for a proactive and experienced Advanced Nurse Practitioner to become part of our supportive, forward thinking clinical team. The role offers 6 - 8 sessions per week and would suit a confident clinician who enjoys working independently while providing high quality, patient focused care. Applicants must be qualified Independent Prescribers, able to prescribe safety and autonomously within their professional scope.

Main duties of the job

Clinical Practice and Supervision

The Advanced Nurse Practitioner will work alongside the clinical team, they will be responsible for assessing, planning, delivering, and reviewing care for patients whose conditions have been pretriaged and fall within their scope of practice such as minor illnesses, chronic disease followups, and selected acute presentations.

Prescribing and Management

The role includes prescribing and reviewing medications in line with evidence based practice, national guidelines, and practice policies, ensuring decisions remain within the practitioners competencies and the prescribing parameters set by the supervising GP. The Advanced Nurse Practitioner will support patients with both shortterm and longterm conditions, using clinical judgement to apply drug and non drug treatment options as delegated by the wider clinical team.

Patient Safety and Referral

The practitioner will provide patients with general health advice and guidance on minor illnesses, while maintaining a high standard of safe and effective clinical practice within their defined level of expertise. When a patient presents with a condition that is complex, unclear, or outside the agreed scope of practice, the Advanced Nurse Practitioner will promptly seek support or escalate care to the supervising GP or appropriate primary or secondary care services.

About us

The Cambridge Practice was established in 2018 following the merger of Southlea Surgery and Victoria Surgery in Aldershot, and has been part of the Aldershot Primary Care Network since 2019.

We are a fully accredited NHS GP Training Practice, with a long history of supporting the education and development of future GPs. We provide a high quality training environment for GP registrars and a range of healthcare students, including nursing, advanced clinical practice, paramedic, and physician associate students. As a training practice, we are regularly reviewed to ensure high standards of patient care and efficient practice management.

Our main site is based within Aldershot Centre for Health, colocated with other healthcare providers, promoting collaborative working. We also operate a branch surgery located to the south of Aldershot, approximately three miles from the main site.

Job responsibilities

The following are the core responsibilities of the Advanced Nurse Practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
  • Clinically examine and assess Patient needs from a physiological and psychological perspective and plan clinical care accordingly.
  • Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
  • Proactively identify, diagnose, and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
  • Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug-based treatment methods into a management plan.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, within scope of practice.
  • Work with Patients in order to support compliance with and adherence to prescribed treatments.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimes, side-effects, and interactions.
  • Prioritise health problems and intervene appropriately to assist the Patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Support Patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.
  • Support and manage health needs of women presenting for family planning, cervical cytology, or sexual health consultation.
  • Assess, identify, and refer Patients presenting with mental health needs in accordance with national and local guidelines.
  • Implement and participate in vaccination and immunisation programmes for both adults and children.
  • Promote and deliver evidence-based care for patients presenting with aural conditions.
  • Meet the needs of patients presenting for opportunistic wound care.
  • As a senior Nurse you will be required to mentor/support/advise junior Nurses and Healthcare Assistants as required.

Working with others:

  • Work independently but also as part of a multi-disciplinary team to ensure the smooth running of the Practice
