Job Summary
As a Practice Nurse at Purbeck Health Centre, youll be playing a key role in supporting patients with long-term conditions, including managing care for elderly and housebound individuals. Youll be an integral member of a dedicated and supportive multidisciplinary team, ensuring that nursing services are delivered effectively to meet the needs of our diverse patient population. This role requires someone with a genuine passion for patient care, the ability to work independently, and a drive to contribute to the ongoing development of our services.
Your responsibilities will span a range of clinical areas, including chronic disease management, health promotion, infection prevention and control, and supporting the practice in clinical policy development. Key ResponsibilitiesPrimary Responsibilities:

Infection Prevention and Control (IPC): Support the IPC lead by assisting with audits, training, and ensuring compliance with IPC guidelines. Help prepare and publish the IPC annual statement.

Chronic Disease Management: Provide care for patients with chronic conditions, ensuring treatment plans are effective and based on current evidence. This includes managing conditions such as diabetes, hypertension, asthma, and COPD, with a focus on prevention and health promotion.

Care for Vulnerable Groups: Develop and deliver care programs for housebound and frail patients, ensuring they receive regular reviews and appropriate interventions.

Health Promotion: Implement and evaluate health promotion activities, including smoking cessation, sexual health advice, and other wellbeing initiatives for patients of all ages.

Patient Education: Provide education to patients regarding their treatment plans, medications, and lifestyle changes, ensuring they are informed and supported in managing their health.

Chronic Disease Clinics: Oversee the chronic disease registers and run clinics for specific conditions, coordinating with GPs and other healthcare professionals for additional support when necessary.

Vaccinations and Immunisations: Administer vaccinations for children and adults, ensuring patients are up-to-date with their immunisation schedules, including the flu, pneumonia, shingles, and HPV vaccines.

Minor Surgery Assistance: Assist GPs in minor surgeries, ensuring patients are cared for before, during, and after procedures.

Mental Health Support: Recognise and assess patients with mental health needs, providing appropriate support and referring to specialist services where necessary.

Clinical Record Keeping: Ensure that patient records are accurately maintained in line with legal and clinical guidelines, including using SNOMED CT codes to ensure easy retrieval of patient data.

Patient Support: Liaise with external agencies and healthcare providers to ensure vulnerable patients are fully supported and receive comprehensive care.
Secondary Responsibilities:

Audit and Quality Improvement: Contribute to the development of audit protocols and review processes, ensuring we are continually improving the care we deliver. Participate in quality improvement initiatives and contribute to reviews of patient care practices.

Team Development: Support the professional development of the nursing team, providing guidance, mentoring, and assisting with training needs analysis. Participate in performance reviews and help colleagues achieve their potential.

Collaboration and Communication: Foster effective teamwork across all areas of the practice. Work closely with GPs, administrative staff, and other healthcare professionals to provide integrated care and ensure that patient needs are met in a timely manner.

Rota and Staffing: Assist in coordinating the nursing teams rota, ensuring that we have the right mix of skills available to meet the needs of our patients at all times.

Policy and Procedure Development: Contribute to the development and review of practice policies and procedures, ensuring that they meet current guidelines and legislation.
Qualifications and Experience

Registered Nurse: You must be a Registered Nurse with an NMC PIN, with previous experience working in a GP practice or primary care setting being an advantage.

Clinical Skills: You should be competent in chronic disease management, administering vaccinations, providing wound care, and carrying out general treatment room tasks (such as ECGs and ear syringing).

Experience in Health Promotion: Experience in providing health education and supporting patients with lifestyle changes is essential.

Team Player: Strong communication skills and a collaborative approach are vital, as youll be working alongside a diverse team to provide the best care for our patients.

Organisational Skills: The ability to work independently, manage your time effectively, and handle a varied caseload is crucial.

IT Skills: Experience with clinical software (such as SystmOne) would be beneficial, but training will be provided if needed.
