Job summary
As the Clinical Coder / Summariser at Belgrave Medical Centre, you will play a key role in supporting the smooth and accurate management of patient health records. The ideal candidate will be detail-oriented, organised, and committed to maintaining the highest standards of clinical data quality.
You will be responsible for reviewing, coding, and summarising patient information to ensure that electronic medical records are accurate, comprehensive, and up to date. This includes interpreting hospital correspondence, identifying relevant diagnoses and procedures, and applying appropriate clinical codes using SNOMED CT and other recognised coding systems.
Your work will contribute directly to the delivery of safe and effective patient care, supporting clinicians in making informed decisions and ensuring compliance with QOF, enhanced services, and other NHS reporting requirements.

Main duties of the job
- Review and summarise incoming and historical patient records, ensuring accurate and relevant data is entered onto the clinical system (SystmOne).
- Apply appropriate clinical codes (SNOMED CT) to reflect diagnoses, procedures, investigations, and key clinical information.
- Process and code correspondence received from hospitals, community services, and other healthcare providers.
- Ensure patient records are kept up to date, accurate, and compliant with national standards.
- Support clinicians by maintaining accurate and easily accessible clinical information to improve patient safety and continuity of care.
- Identify data that contributes to QOF, Enhanced Services, and other reporting frameworks.
- Collaborate with administrative and clinical teams to ensure timely management of patient documentation.
- Maintain confidentiality and comply with GDPR, NHS Data Security, and practice policies at all times.
- Contribute to ongoing data quality improvement initiatives within the practice.
