Elmwood Family Doctors is a large, modern General Practice located across two sites in Huddersfield (Holmfirth and Meltham), rated as outstanding by CQC.
We are looking to recruit an enthusiastic and motivated Lead Practice Nurse to join our supportive and friendly team and lead a team of Practices Nurses, Health Care Assistants, Trainee Nurse Associates and a GP Assistant.

Main duties of the job
The successful candidate will be responsible for the management of the practice nursing team, consisting of 3 nurses, 2 TNA'S, 5 HCA'S and a GP Assistant.
As well as being responsible for several clinical areas such as infection prevention and control, health promotion, chronic disease management health prevention, well women and well man clinics, as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

The following are the core responsibilities of the Lead Practice Nurse:
- To act as the Infection Prevention Control (IPC)
lead and to work to the IPC Policy providing audits, training, support and
evidence of compliance to the practice manager
- To lead on and deliver chronic disease management for patients
- To lead the team in providing routine nursing care to
patients in accordance with clinical based evidence, NICE and the NSF
- To lead on the practice's vaccination and immunisation programmes for adults and children
- To lead on cytology
- To lead on wound care management and dressings
- To prescribe medication for patients in
accordance with PGDs and own scope of practice
- To review patient medications to enhance
compliance
- To provide guidance to patients on the use of
prescribed and over-the-counter medications
- To develop, implement and embed health promotion and wellbeing programmes
- To develop, implement, embed and participate in
well woman and man clinics
- To support the team in delivering anti-coagulation clinics
- To support
patients in the use of their prescribed medicines or over-the-counter medicines
(within own scope of practice)
