Job summary
Liscard Group Practice is looking for a Practice Nurse to join our multi-disciplinary team on a part time basis.
20-30 hours/week over 3-5 days (we are open Mon-Fri 8:00-18:30) - negotiable.
A prescribing qualification would be advantageous but is not essential.
Main duties of the job
The practice nurse will be responsible for a number of clinical areas such as health promotion and chronic disease management.
There will be a requirement to undertake occasional home visits, if and when required, to undertake chronic disease management reviews and a range of other nursing duties on patients who are housebound.

We are a friendly, well-organised 2 Partner Practice serving around 4600 patients.
We have been based at Victoria Central Health Centre since 2009. It is a modern, purpose built centre with plenty of on-site parking.

Provide
routine nursing care to patients as required in accordance with clinical-based
evidence, NICE and the NSF.
Provide chronic disease clinics, assessment, screening services, health education advice and vaccinations. This may include (but not restricted to);
- Annual Diabetic Reviews
- Annual Asthma Reviews
- Annual COPD Reviews
- Cervical Smears
- Urinalysis
- Phlebotomy
- Dressings
- Removal of sutures / clips
- Contraception advice
- Routine, travel and seasonal vaccinations/immunisations
for both adults and children
- New patient health screening / NHS Heath Checks / general health advice
- ECG
- Hypertension Reviews
Maintain
accurate clinical records in conjunction with legislation
Work In collaboration
and support HCA and Phlebotomy team as required.
Assess, diagnose and treat patients in
practice premises or their own home who require medical attention and/or
refer to appropriate agency.
Deliver
opportunistic health promotion where appropriate
