Job summary

A brand new, exciting opportunity has arisen for an experienced Advanced Nurse Practitioner to work at our city centre practice in Chichester, full or part-time.

The primary focus of the work will be working with GPs to deal with our care home patients (130 beds).

In addition, to assist our acute duty day team, providing medical care, assessments, evaluation and treatment for patients .

Depending on your skills base, we have flexibility to tailor the role further around chronic disease management.

We have 13,500 patients cared for by a team of 10 GPs, Practice Nurses, HCAs, Clinical pharmacist and Paramedics. We are a longstanding training practice, with a track record of professional development. We value team working, resilience and mutual support very highly and a have a unique working structure to support these priorities.

Prescribing qualification preferred. Must have a master's degree level in the relevant areas of expertise. We can be flexible around the hours of work, whether part-time or full-time.

Main duties of the job

1. Care homes - first port of call for clinical queries from our 3 care homes (130 beds). Advice and treatment plans. May be over the phone or in person.

2. Assist duty team (GP and paramedic) where required, providing care, assessments and treatment plans for our non care home patients.

3. Chronic disease management (eg hypertension, diabetes, AF) - the role can be tailored to your skills base with the aim of optimising the healthcare of such patients.

About us

We are a large practice of 13,500 patients in Chichester city centre. We have 3 GP partners, plus 7 others GPs, a team of 4 Practice Nurses, 2 HCA's, a clinical pharmacist and 2 paramedics. We work from a newly refurbished listed building and have a small branch surgery in Bosham. We are also a dispensing practice for those patients living in rural areas.

We are a GP training practice and have a long history of developing our staff, whether clinical and non-clinical. We highly value team working and mutual support and have a unique working arrangement in our building to facilitate our priorities.

Job summary

1. 1.Care homes - first port of call for clinical queries from our 3 care homes (130 beds). Working closely with our care co-ordinators on managing home patients, either over the phone or visiting

  • New resident care plans following hospital discharge incl RESPECT
  • Deterioration of patients including dementia
  • Prescription queries
  • Minor illness chest infections, UTIs etc
  • Weekly MDT with each home
  • Liaison with outside agencies eg community nursing team

2. Assist duty team (GP and paramedic) where required, providing care, assessments and treatment plans for our non care home patients.

3. Assisting with chronic disease management (eg hypertension, diabetes, AF). Can involve auditing areas of work to prioritise and optimise the healthcare of such patients.

Key responsibilities

Clinical practice

  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
  • Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
