Job summary

We are looking to recruit a suitably qualified Care Coordinator under NHS Englands Additional Roles Reimbursement Scheme (ARRS) to work in our multidisciplinary team (MDT). This is a great chance to join our quickly developing Primary Care Network (PCN) working across our six member GP practices in the East Riding of Yorkshire with a particular focus for this vacancy on patients in care homes.

Main duties of the job

This is a non-clinical role which requires strong, proactive, organisational and communication skills. Care Coordinators are a key contact point for patients and care homes. Our Care Coordinators will work closely with PCN, care homes and practice teams to link patients with the help and support they need. The successful Care Coordinator candidates will therefore work proactively with people, including the frail/elderly and those with long-term health conditions, and will coordinate and navigate care and support across services for them, including making referrals where appropriate

Harthill Primary Care Network (PCN) is a network of six NHS GP Practices within the Haltemprice and Market Weighton areas of the East Riding of Yorkshire which includes practices in Cottingham, Willerby, Anlaby, Hessle, North Ferriby and Market Weighton. We are one of seven PCNs across the East Riding and cover a registered population of over 35,000 patients

Proactively identify and work with cohorts of patients to deliver personalised care. Support patients to utilise decision making tools to prepare for shared decision-making conversations. Working holistically, bring together their identified care and support needs and explore options to meet these within a single Personalised Care and Support Plan (PCSP). Help people to manage their needs by answering their queries, making and managing appointments, providing good quality written or verbal information to inform decision making and enhancing their knowledge, skills and confidence. Support people to take up employment and training and to access appropriate benefits through referral to the Social Prescribing Link Workers. Connect patients with tools to support them, such as self-management education, peer support and the adoption of healthy behaviours. Essential

  • Enrolled/in progress/qualified/willing to be enrolled in appropriate training as set out by the Personalised Care Institute (PCI)
  • Demonstrable understanding of the personalised care approach
  • Understands how the NHS works including primary care and PCNs
  • To understand and in practice apply Caldicott Principles, Data Protection Act 2018 and appropriately protect an individuals personal data, disclosing where appropriate only
  • Awareness of the wider determinants of health and its impact
  • Understands the needs of older people/adults with disabilities/long terms conditions particularly in relation to promoting their independence

Desirable

  • Has safeguarding knowledge either level 2 or the equivalent experience

  • To deliver outcomes based on set targets
  • Liaise with other colleagues to promote the success of the PCN and its Partner organisations in primary care
  • Work to perpetuate a culture that is transparent and open
  • Has collated data and used tools to demonstrate the impact of services
  • Recognises and works within limits of competence, seeking advice when needed
  • Awareness of and has well-developed IT skills to facilitate quick and confident learning of new systems to support the management of patients in a primary care setting (including Microsoft packages)
  • Working well with others to deliver goals
  • Being motivated to deliver excellent services to support Primary Care in our region
  • Experience of working directly in a care coordinator role, in adult health, social care, learning support or public health/health improvements
  • Experience of working in health, social care and other support roles in direct contact with people, families or carers (in a voluntary or paid capacity)
  • Experience of working in multidisciplinary professional team environments

Desirable

  • Knowledge of SystmOne and/or EMIS clinical systems
  • Experience of working directly in a care coordinator role, in adult health, social care, learning support or public health/health improvements
